Spontaneous evisceration of abdominal viscera is a rare complication of incisional hernia which could pose a serious threat to life if intervention is delayed. We report a case of a 62-year-old female with a history of curative resection for stage 1 endometrial adenocarcinoma 3 years ago, presenting with spontaneous evisceration of incisional hernia with strangulation of small bowel. Immediate resuscitation followed by emergency surgery was mandatory. During surgery, priority should be given to release the strangulation as soon as possible and the type of repair would depend on the viability of the bowel and the anatomy of the incisional hernia.
Carcinoid tumours are neuroendocrine tumours which arise from the enterochromaffin cells in the gastrointestinal and bronchopulmonary systems. The presentation of multiple gastrointestinal carcinoids with jejunal intussusception is rare, and the diagnosis may be challenging. A 49-year-old patient with adult onset bronchial asthma presented with pain around the umbilical region for 1-day duration. Physical examination revealed only mild abdominal tenderness. Abdominal computed tomography revealed small bowel intussusception with two separate highly vascular tumours arising in the small bowel mesentery. Exploratory laparotomy was done, with resection of the tumours arising from the small bowel mesentery and the proximal jejunum causing the intussusception which were excised. Histopathological diagnosis confirmed the presence of a Grade 1 carcinoid tumour of classic type. After surgery, he had an uneventful recovery and was asymptomatic. Carcinoid tumours are a very rare cause of adult intussusception. So far, there have been only two reported cases of jejunal intussusception secondary to carcinoid tumours. These will require a combination of surgical intervention and systemic therapy in selective cases for complete management.
Introducción: Los hospitales de día suponen una alternativa asistencial a la hospitalización convencional mejorando la eficacia de la asistencia sanitaria. Desde nuestro punto de vista, la nefrología se beneficiaria de esta modalidad de atención, ya que el paciente con enfermedad renal crónica tiene unas necesidades concretas de cuidado que conllevan al profesional de enfermería a generar una atención integral e individualizada. Por este motivo surge en nuestro servicio la creación de la Unidad Nefrológica de Atención Continuada.
Objetivo: Presentar nuestra experiencia en la implantación y desarrollo de la Unidad Nefrológica de Atención Continuada en el Hospital Universitario del Henares.
Material y Método: Estudio descriptivo retrospectivo de la actividad realizada en la Unidad Nefrológica de Atención Continuada entre enero-junio de 2017. Desarrollo y puesta en marcha de las distintas funciones y competencias profesionales.
Resultados: En este periodo se asistieron a un total de 874 pacientes, con una media de 145,6 visitas/mes. De todas estas visitas, se atendieron 474 pacientes en programa de Diálisis Peritoneal (55% del volumen total), 149 pacientes con enfermedad renal crónica (16%), 245 pacientes con enfermedad renal crónica avanzada (18%) y 11 pacientes en tratamiento conservador (1%). Se realizaron diferentes técnicas, bien programadas en la agenda electrónica o a demanda.
Conclusiones: La implantación de la Unidad Nefrológica de Atención Continuada, de forma estructurada y planificada, es una alternativa válida y necesaria en nuestro sistema sanitario. Permite aseguramos un abordaje integral de los pacientes renales y alcanzar una continuidad de cuidados con menor coste sanitario.
