The aim of this study was to compare computed tomography (CT) scans of chest and lung function among patients with Human T-Lymphotropic Virus Type 1 (HTLV) with and without HTLV-1 associated myelopathy/tropical spastic paraparesis (HAM/TSP). In this cross-sectional study performed between January 2013 and June 2016, we included 48 patients with HAM/TSP (19 women and 11 men) and without HAM/TSP (12 women and 6 men). We compared CT findings and lung functions of these groups. Patients who had HAM/TSP had abnormal CT findings (P = 0.000), including more frequent bronchiectasis (P = 0.049), parenchymal bands (P = 0.007), interlobular septal thickening (P = 0.035), and pleural thickening (P = 0.009). In addition, neither patients with HAM/TSP (9/30; 30%) nor the controls (0/18; 0%) had obstructive or restrictive lung disease (P = 0.009). HTLV diagnosis should be considered in all patients with abnormal CT findings in whom no other cause is apparent. It is important to remember that lung disease increases the rates of morbidity and mortality in developing countries.
Resumo: Introdução: O cotidiano é um fenômeno complexo, multidimensional e processual, no qual todas as pessoas estão inseridas, é a medida da sucessão da vida humana, sendo a base constituinte da vida, promovendo o(s) sentido(s) de continuidade da existência. Dentre as doenças infantis, as neoplasias destacam-se por sua alta incidência e repercussões na vida e no cotidiano da criança e de seus familiares ou responsáveis, sendo comum que crianças com neoplasia e seus acompanhantes vivenciem a hospitalização. Objetivo: Considerando a relevância e repercussões do cotidiano vivido para a saúde e bem-estar, objetivamos desvelar os significados da vivência do cotidiano hospitalar para mães de crianças com diagnóstico de neoplasia. Método: Trata-se de uma pesquisa qualitativa, com enfoque fenomenológico, na modalidade análise do conteúdo, a partir da aplicação de entrevistas semidirigidas com nove mães, em um Hospital da Rede Pública. Resultados: Os resultados revelaram que o cotidiano hospitalar pode ser permeado pelo desconforto físico, angústia; medos, incertezas, dificuldades de adaptação às regras, ambiente e procedimentos hospitalares; pela saudade, afastamentos de pessoas, contextos e ocupações significativos; e pelo não fazer ou fazeres vinculados ao cuidado com a criança. Todavia, as pessoas podem buscar (re)significar o cotidiano, através de vivências ocupacionais e relações de apoio entre acompanhantes, o que pode contribuir para a percepção de bem-estar e aprendizagem decorrente desta experiência. Conclusão: Portanto, os sentidos desta vivência e a forma como esta pode interferir na saúde e bem-estar das pessoas devem ser inquietações de terapeutas ocupacionais em hospitais. A proposta deste trabalho consistiu em compartilhar um serviço de pronto-atendimento psicológico às pessoas que vivenciam uma perda significativa, seja por morte de uma pessoa de vinculação importante, como a de um familiar ou amigo, óbito de um paciente ou perda da saúde quando do adoecimento e/ou hospitalização, entre outros, e discutiu os resultados de tal serviço. O enfoque metodológico baseou-se na análise do processo de estruturação e implantação de um serviço dessa especificidade em um hospital público universitário, onde os autores desenvolvem a assistência aos que vivenciam esses processos de perda. Os resultados apontaram a importância da oferta desse serviço, que foi demandado tanto por aqueles que vivenciam o luto pela perda de saúde ou pela morte de uma pessoa significativa como por profissionais de educação e saúde.
Introduction: Palliative care is a set of approaches to improve the quality of living for people who are out of the possibility of cure of a particular illness. Generally, they demand from family members the task of being caretakers, implying in their day-today occupations. The occupations are the various daily actions carried out by individuals, in groups and in the communities to which they belong, to fill the time and bring meanings and purposes to life. The occupational form understands what people do, how they do it, and in what circumstances they do. It is understood that caregivers of people who are in palliative care have a peculiar way of occupying themselves and with characteristic conditions. Objective: To understand the form of occupations of the main caregivers of people in oncological palliative care. Method: This is a qualitative research, carried out at a reference hospital in palliative care in the Northern Region of Brazil, at the Oncology Palliative Care Clinic (CCPO). An open interview was conducted with 20 family caregivers. Results: Outstanding changes in routine organization and dedication to caring for the loved one in palliative care. It also revealed that being a caregiver led to the difficult task of including, maintaining, and excluding day-today occupations. Conclusion: The study highlights the implications in the occupational repertoire in the condition of being a caregiver in palliative care and how the occupation is developed and drawing attention to the need for health professionals to direct their attention also to caregivers.
