This article analyses a m ajor water crisis in a Third W orld city, and its resolution. The city is M onterrey, M exico. The crisis occurred in the 1970s and 1980s, and its resolution came in the late 1980s and 1990s. This study m akes the following key points. (1) W ater infrastructure development occurs within m icro-as well as m acroeconom ic and political contexts. (2) Equitable infrastructure planning can occur only w hen the political contexts are favourable, even if the economic contexts are not. (3) Citizens who are not part of the form al decision-m aking structure (such as low -income w omen) can in¯uence policy developmentand prioritization of investments for hydraulic infrastructure.

This article aims to demonstrate how people resolve the conflicts that arise when faced with uncertain choices and how midwives need to respect and respond to those decisions. Midwives must be aware of their ethical and legal responsibilities when offering screening tests and when supporting women in their decisions.
