BackgroundIntestinal parasitic infections represent a public health problem in Tajikistan, but epidemiological evidence is scarce. The present study aimed at assessing the extent of helminths and intestinal protozoa infections among children of 10 schools in four districts of Tajikistan, and to make recommendations for control.MethodsA cross-sectional survey was carried out in early 2009. All children attending grades 2 and 3 (age: 7-11 years) from 10 randomly selected schools were invited to provide a stool sample and interviewed about sanitary situation and hygiene behaviour. A questionnaire pertaining to demographic and socioeconomic characteristics was addressed to the heads of households. On the spot, stool samples were subjected to duplicate Kato-Katz thick smear examination for helminth diagnosis. Additionally, 1-2 g of stool was fixed in sodium acetate-acetic acid-formalin, transferred to a specialised laboratory in Europe and examined for helminths and intestinal protozoa. The composite results from both methods served as diagnostic 'gold' standard.ResultsOut of 623 registered children, 602 participated in our survey. The overall prevalence of infection with helminths and pathogenic intestinal protozoa was 32.0% and 47.1%, respectively. There was pronounced spatial heterogeneity. The most common helminth species was Hymenolepis nana (25.8%), whereas the prevalences of Ascaris lumbricoides, hookworm and Enterobius vermicularis were below 5%. The prevalence of pathogenic intestinal protozoa, namely Giardia intestinalis and Entamoeba histolytica/E. dispar was 26.4% and 25.9%, respectively. Almost half of the households draw drinking water from unimproved sources, such as irrigation canals, rivers and unprotected wells. Sanitary facilities were pit latrines, mostly private, and a few shared with neighbours. The use of public tap/standpipe as a source of drinking water emerged as a protective factor for G. intestinalis infection. Protected spring water reduced the risk of infection with E. histolytica/E. dispar and H. nana.ConclusionsOur data obtained from the ecological 'lowland' areas in Tajikistan call for school-based deworming (recommended drugs: albendazole and metronidazole), combined with hygiene promotion and improved sanitation. Further investigations are needed to determine whether H. nana represents a public health problem.
In the Sahel, between Mauritania and Somalia including Northern Kenya, about 20-30 million people live as mobile pastoralists. The rhythm of their migration follows the seasons and the availability of resources such as water, pasture and salt. Despite their high exposure to zoonoses and problems caused by extreme climatic conditions, mobile pastoralists are virtually excluded from health services because the provision of social services adapted to their way of life is challenging. In cooperation with various partners in the region, the Swiss Tropical and Public Health Institute has been active in research and development in the Sahel for 15 years. Based on the perceived needs of mobile pastoralists and the necessities of development, interdisciplinary research has considerably contributed to better understanding of their situation and their problems. Close contact between humans and livestock necessitates close cooperation between human and animal health specialists. Such useful approaches should be continued and extended.
BackgroundDemographic information is foundational for the planning and management of social programmes, in particular health services. The existing INDEPTH network surveillance sites are limited to coverage of sedentary populations. Including mobile populations in this approach would be expensive, time consuming and possibly low in accuracy. Very little is known about the demography of mobile pastoralists and their animals, so innovative approaches are urgently needed.ObjectiveTo test and evaluate a mobile demographic surveillance system for mobile pastoralist households, including livestock herds, using mobile phones.DesignMobile pastoralist camps were monitored (10 for 12 months and 10 for 18 months) using biweekly mobile phone calls with camp leaders and their wives to conduct interviews about the households and livestock. The collected information was validated through personal visits, GPS data and a livestock demographic model.ResultsThe study showed the feasibility of mobile phone surveillance for mobile pastoralist camps, providing usable, valid information on human and livestock population structures, pregnancy outcomes and herd dynamics, as well as migration patterns. The approach was low-cost and applicable with the existing local resources.ConclusionDemographic surveillance in mobile populations is feasible using mobile phones. Expansion of the small-scale system into a full mobile demographic surveillance system is warranted and would likely lead to improved planning and provision of human and animal health care.
