Proses pengumpulan data dalam penelitian survey merupakan hal yang sangat penting. Penelitian berbasis survey membutuhkan instrumen pengumpulan data berupa kuesioner. Selama ini peneliti atau surveyor melakukan pengumpulan data secara manual yaitu dengan mencetak kuesioner dan menyebarkannya dengan mendatangi satu persatu responden. Hal ini memakan waktu yang lama dan biaya yang besar sehingga menjadi masalah utama yang dihadapi dalam pengumpulan data dengan kuesioner. Untuk mengatasi masalah tersebut dibutuhkan suatu aplikasi yang dapat membantu surveyor dalam melakukan survey secara online. Penelitian ini bertujuan untuk merancang dan membangun aplikasi kuesioner online berbasis web dengan meggunakan metode waterfall dengan tahapan analysis, design, implementation, testing dan maintanance. Pemodelan sistem dan basis data meggunakan Data Flow Diagram (DFD) dan Entity Relationship Diagram (ERD). Bahasa Pemrograman yang digunakan yaitu, HTML, PHP, CSS, Java Script, dan untuk databasenya menggunakan MySQL. Skala pengukuran yang digunakan dalam perancangan kuesioner menggunakan skala likert dan skala guttman. Aplikasi ini memiliki 3 hak akses yaitu, admin, surveyor dan pengunjung/responden. Dengan adanya aplikasi diharapkan ini proses pengumpulan data dengan kuesioner dapat lebih efektif dan efisien.
AbstrakPada Unit Pelayanan Teknis Kesatuan Pengelolaan Hutan Produksi (UPT-KPHP) Kapuas Tengah Unit XI Kapuas Tengah Unit XI dalam melaksanakan tugasnya masih belum menerapkan sistem informasi untuk memfasilitasi pengolahan data pegawai, pengolahan Daftar Urut Kepangkatan, pengolahan Sasaran Kerja Pegawai (SKP), pengolahan Penilaian Kerja Pegawai (PKP), serta Kenaikan Pangkat untuk jabatan struktural. Hal tersebut menimbulkan beberapa masalah bagi UPT-KPHP Kapuas Tengah Unit XI seperti penyimpanan informasi dalam bentuk cetakkan pada kertas, pencarian suatu berkas yang disimpan pada rak-rak berkas, kerusakan atau kehilangan arsip, dan sarana penyimpanan arsip. Sehingga, penyelesaian dari masalah ini adalah dibangunnya sebuah sistem informasi kepegawaian berbasis website bagi UPT-KPHP Kapuas Tengah Unit XI. Sistem Informasi Kepegawaian dirancang dan dibangun metodologi waterfall menurut Ian Sommerville. Adapun tahapannya yaitu Requirements Definition, System dan Software Design, Implementation and Unit Testing, Integration and System Testing, Operation and Maintenance. Pada tahap Requirements Definition dilakukan pembuatan flowchart. Pada tahap System dan Software Design dilakukan pembuatan Data Flow Diagram (DFD) dan Entity Relational Diagram (ERD). Metode testing yang digunakan website ini adalah metode Blackbox. Sistem informasi kepegawaian ini dapat digunakan untuk memfasilitasi pembuatan SKP dan PKP oleh pegawai dan pimpinan, mengajukan usulan pangkat untuk jabatan structural, penyusunan Data Urut Kepangkatan, dan membantu staff administrasi mengelola data kepegawaian. Thus, the solution to this problem was the construction of a website-based personnel information system for Central Kapuas Unit XI.The personnel information system is designed and built a waterfall methodology by Ian Sommerville. There are some stages; Requirements Definition, System and Software Design, Implementation and Unit Testing, Integration and System Testing, Operation and Maintenance. Requirements Definition stage is conducted by creating a flowchart. While in System and Software Design stage, there is creation of Data Flow Diagram (DFD) and Entity Relational Diagram (ERD). The scholarship is a development program obtained by the students to be able to enroll their studies, in the form of financial assistance. At the Economics Institute of Palangka Raya, there is a BBP-PPA Scholarship to help the students who are financially disadvantaged. In selecting the scholarship awardees, Economics Institute of Palangka Raya experienced difficulties due to a large number of prospective scholarship awardees. Because of that, the Economics Institute of Palangka Raya disposes to accelerate the selection process by using one criterion, that is by grading the students� GPA. This process might be inaccurate, indeed the selection process will be much quicker by that GPA rank criterion. Yet it is inappropriate considering other several criteria that have to be considered carefully as well. Thus, the solution to this problem is to establish the Decision Support System to select the BBP-PPA scholarship awardees which are web-based. The Decision Support System (DSS) uses the Technique for Order Preference Method by Similarity to Ideal Solution (TOPSIS). In developing the DSS, it will apply Waterfall software development methodology as Ian Sommerville�s. The software used in building this website is Notepad ++, SQLyog, and XAMPP. This system can run well as black box testing on the system.

Designing Website Information Systems Practicum is developed with the purpose as an information medium that can provide information such as schedule of data practicum and registration practicum for college students, can help lab assistant in registering the lab, managing attendance list and practicum scores, help head of the laboratory in monitoring the implementation of practicum, and can assist administrators in making the practicum schedule.
Kartu Rencana Studi is an important activity carried out by students to determine the courses to be taken in the following semester, the process of filling out Kartu Rencana Studi is done one week before the lecture is really active and if students do not fill out in accordance with applicable regulations, the student concerned is considered on college leave. In the Department of Non-School Education, the University of Palangka Raya to fill in Kartu Rencana Studi still uses a manual method where students consult with their academic supervisor to ask for approval of the courses to be programmed in the semester that will take place. In the implementation of this filling process can cause problems, one of which is the delay of students in completing Kartu Rencana Studi, it is difficult to control the courses taken by students, not all courses that want to be programmed in accordance with the courses issued and there are some specific subjects who experience conflicts on the same scheduled day and hour make students have to readjust the schedule and courses to be taken with the available schedule. This research design made up of two stages: (1) data collection and analysis conducted by means of literature study and observation, (2) applying the waterfall method already modified with the stages is system analysis, design, implementation and testing. The implementation uses PHP and Mysql as a database to store data. Furthermore, at this testing stage using the Blackbox method.
