Latar belakang: Telemedicine telah mengubah lanskap sistem komunikasi kesehatan. Pola perubahan ini berkaitan erat dengan budaya maupun pola lain yang niscaya harus diikuti oleh stakeholder penyedia layanan kesehatan. Perubahan ini bukan hanya mengenai kemampuan memahami telemedicine sebagai teknik penggunaan teknologi, namun juga kemampuan untuk memberikan pemaknaan atas relasi komunikasi termediasi dalam dunia kesehatan.Metode: Penelitian ini berupaya untuk mengaji interpretasi stakeholder kesehatan dalam memaknai dan memahami pengembangan program komunikasi kesehatan berbasis teknologi telemedicine “TEMENIN”, dengan menggunakan pendekatan fenomenologi Hans-Georg Gadamer.Hasil: Terdapat 6 pola antar kasus interpretasi stakeholder kesehatan terhadap telemedicine. (1) modernisasi pelayanan telemedicine. (2) Problem teknikalisasi pembangunan. (3) Otoritas, kewenangan, dan kompetensi sumber daya program. (4) Nalar ekonomis pelayanan kesehatan. (5) Perubahan relasi dokter-pasien. (6) Regulasi maupun perlindungan hak pasien.Simpulan: Adapun kesimpulan penelitian ini di antaranya adalah: pertama, permasalahan sosial hingga moral etis komunikasi pelayanan kesehatan menjadi dampak dari pengembangan telemedicine yang dipahami ansich sebagai permasalahan infrastruktur teknis, seperti halnya keterbatasan jangkauan pengamatan klinis subjek pada realitas teknologi yang tertangkap secara artifisial. Kedua, adanya perubahan aspek intensionalitas dan intuisi subjek dalam praktik komunikasi kesehatan berbasis telemedicine. Perubahan ini mendorong terjadinya reduksi horizon kesepahaman tentang penghayatan subjek pasien terhadap realitas. Ketiga, aspek komunikasi, pemahaman dan pemaknaan subjek dalam pengembangan telemedicine dipengaruhi oleh tradisi dan otoritas. Komunikasi berbasis teknologi yang berupaya untuk mengatasi kesenjangan, di sisi lain justru membuka ruang kesenjangan baru. Keempat, hubungan sarana menjadi kecenderungan pemahaman dalam membangun relasi dan intensi subjek saat menggunakan telemedicine. The change pattern is closely related to culture and other patterns that must be followed by stakeholders of health service providers. This change is not only about the ability to understand telemedicine as a technique of using technology, but also the ability to give meaning to mediated health communication system. Method: This study examines the subjective experience of health stakeholders in interpreting and understanding the program development of the telemedicine "TEMENIN", using the phenomenological approach of Hans-Georg Gadamer.Result: This study found that there were 6 patterns between cases of interpretation of health stakeholders on telemedicine. (1) modernization of telemedicine services. (2) The problem of technicalization of development. (3) Authority and competence of program resources. (4) Economic reasoning for health services. (5) Changes in the doctor-patient relationship. (6) Regulation and protection of patient rights.Conclusion: (1) The development of telemedicine has created social to ethical moral problems of health care communication, which is understood as the limited range of clinical observations of subjects in artificially captured technological realities. (2) There is a change in the aspect of intentionality and intuition of the subject in the practice of telemedicine-based health communication, and encourages a reduction in the horizon of understanding. (3) Aspects of communication, understanding and meaning of subjects are influenced by tradition and authority.
