Introduction: One of the goals of the health system is to ensure that people are healthy. Reaching this goal is only possible through the provision of the proper services. The satisfaction of patients with the quality of health care services is used as a questionare to investigate the effectiveness of hospital services concerning consumption management and patient care quality. It assists managers and the decision-making network to identify the strengths and weaknesses of the system. Objective: This study aimed to investigate the satisfaction of hospitalized patients in educational and medical centers of Rasht City, Iran with the “Health Care System Reform Plan” in 2019. Materials and Methods: The present study is an analytical cross-sectional study. A total of 395 patients hospitalized in educational and medical centers of Rasht City were selected based on the stratified random sampling method and according to the inclusion criteria. The study data were collected by a questionnaire examining patients' satisfaction with the “Health Care System Reform Plan” developed by the Treatment Deputy of Guilan University of Medical Sciences. The questionnaire has 37 questions in 5 areas of patient’s satisfaction with hoteling, hospital bills reduction program, resident physician program, notification program, the program for the promotion of normal childbirth. Data analysis performed using descriptive statistics indicators and Mann-Whitney U, Kruskal Wallis, and logistic regression model. Results: The results showed the mean±SD of samples' age was 43.51±18 years. The majority (23.8%) of the samples were less than 30 years old. Moreover, there was a significant difference between satisfaction score of the Plan (P = 0.009) in terms of gender, hospital bills (P = 0.001), resident physicians (P = 0.009), and notification program (P = 0.032). Also, men had a higher overall score than women. The findings of this study showed that the mean ± SD satisfaction was equal to 82.8 9.1 with a minimum score of 39.71. In the logistics model, only two variables of the hospital (P = 0.017) and job (P = 0.001) were considered as the most important factors related to the overall satisfaction with the “Health Care System Reform Plan.” Conclusion: Based on the results of the study, patients' satisfaction with the implementation of the “Health Care System Reform Plan” was at the acceptable level and various factors have contributed to this level of satisfaction.
Stroke is the most prevalent debilitating neurological disease in adults, which damages the motor system. Therefore, rehabilitation programs are essential to improve the condition of these patients. One of the novel rehabilitation methods is Mirror Therapy (MT). Objective: This study aimed to evaluate the effect of MT on the walking ability of patients after stroke. Materials and Methods: In this clinical trial, 93 patients with stroke were studied in one of the physiotherapy centers in Rasht City, Iran, in 2016. They were assigned into three groups using block randomization technique. In the MT and nonreflective groups, the rehabilitation exercises besides routine rehab program were administered in twenty 15min sessions every other day in front of the mirror (reflective plane for the MT group) and wood (nonreflective plane for the nonreflective group). The control group received only routine physiotherapy program. The walking ability of the samples was evaluated by the Functional Ambulation Category (FAC) test before and after the intervention. For analyzing the collected data, we used the Chi-squared test, ANOVA Kruskal-Wallis test, Friedman test, Mann-Whitney U test, and Generalized Estimating Equation (GEE) model. Results: There were no significant differences between the three groups before the intervention in terms of FAC test score and confounding variables. But all three groups showed a significant difference compared to their pretest scores (P<0.05). Also, the MT group showed more significant improvement in walking ability compared to the other two groups. Conclusion: Mirror therapy, as a simple and client-centered treatment, combined with the routine rehabilitation programs, can improve patients' ability to walk after a stroke.
Introduction:Among important factors in hospitalized patients' satisfaction is respect for their privacy, which can accelerate the healing process and reduce hospitalization time. This study aimed to determine the most important barriers of patients' privacy from nurses' view point in educational-therapeutic centers affiliated to Gilan University of Medical Science in Rasht city in 2015-2016. Methods and Materials:This is a cross-sectional, descriptive, analytical study that was conducted on 186 nursing managers chosen by census method and 230 nurses chosen by stratified random method. Data were collected using reliable and valid researcher-made questionnaires. SPSS21 data analysis software and descriptive statistics (mean, standard deviation) and Factor Analysis were performed. Results:The results showed that through Factor Analysis of the 23 words of the questionnaire, the most important barriers to patient privacy in the four areas were as follows: management domain with special value (7.6 ) and variance percent predicted 24.7% 2) equipment and facilities and manpower domain (environmental domain) with special value of 3.54 and variance percent predicted 20.08%,3) individual-caring domain with special value of 1.82 and variance percent predicted 13.85% ,4) cognitive domain with special value of 1.37 and variance percent predicted 12/9% and in the range of 71.73% of total variance can predict hidden obstacles privacy. Conclusion:The results showed that the management domain is the most important areas effective in noncompliance with patient privacy. It is suggested that retraining courses be held in this area so that nursing managers get more familiar with it.
