Background:Transareola single-site laparoendoscopic thyroidectomy (TASSET) is a rapidly advancing minimally invasive procedure. The purpose of this study was to evaluate the learning curve for TASSET.Subjects and Methods:Forty-five consecutive patients were prospectively divided into group 1 (initial phase), group 2 (intermediate phase), and group 3 (advanced phase) according to their surgical order (15 patients in each group). The operative time, operative blood loss, duration of hospital stay, postoperative pain, and postoperative complications were compared using phases.Results:Statistically significant differences were observed in the different learning phases, among operative time (P<0.05), operative blood loss (P<0.05), hospital stay (P<0.05), and postoperative pain (P<0.05). The postoperative complication rate was low (3/45).Conclusions:Learning curve of the TASSET are improved synchronized at different phases and technical indicators. The establishment of operative space take longer time to skilled master.
Background: Among the complications related to colostomy, variceal hemorrhage caused by cirrhosis is a rare case. The occurrence of peristome variceal hemorrhage should be rapid and effective treatment, otherwise it will result in hemorrhagic shock, repeated stoma bleeding, systemic organs deterioration and other adverse events. Case presentation: In this case of liver cirrhosis, Stoma varices and repeated bleeding due to decompensated cirrhosis was performed by local compression and suture therapy but failed with rebleeding. Being combined with abdominal enhanced CT images,Stoma varices are derived from the branch of inferior mesenteric vein, whose above trunk is located and highly ligated and vein balls are stripped. Meanwhile, the Stoma below abdominal wall varicose veins is sutured by intracutaneous way, which could achieve satisfactory hemostasis in intra-operation. Abdominal enhanced CT images prompted that peristome varicose veins are disappeared. The skin around the stoma is flat and the mucosa is ruddy when changing the bag in post-operation. Conclusion: The report that our team reduced the risk of peristomal variceal bleeding by blocking the main variceal vein through a small incision under local anesthesia may open up another treatment strategy for such patients.
From the perspective of relevance theory, this paper studies and analyzes the image translation and semantic selection in 29 English translation versions of willow in 15 representative Tang poems. Under the framework of relevance theory, translation is divided into two processes, one is the ostensive-inferential process to search for relevance, and the other is to adjust the cognitive environment so that the target reader obtains the contextual effect of the original text reader. In the imagery translation of willow in Tang poetry, the translator is both the receiver and the communicator. In the translation, sufficient clues should be set to help the reader to capture optimal relevance in the target text and the original author's informational intention and communicative intention. The authors classified the willow images into five types. Due to the differences in the cognitive environment between Chinese and English readers, this paper puts forward two translation principles and two suggestions to translate willow images in Tang poetry to realize the mutual manifestness and optimal relevance. The authors hold that relevance theory is helpful in the translation of Tang Poetry.
Breast cancer (BC), a common malignancy, is intractable at present. Cuprous oxide nanoparticles (Conps) are new nanoparticles crucial in inhibiting tumor progression according to early studies, but their role in BC remains unclear. Field emission-scanning electron microscopy (FESEM) and field emission–transmission electron microscopy (FETEM) were used for the morphological observation of Conps, and energy-dispersive X-ray spectroscopy was used for their elemental composition analysis. Multipoint nitrogen adsorption was used to determine the Brunauer–Emmet–Teller of Conps, and their mean hydrodynamic size and zeta potential in water and intact cell medium were determined by dynamic light scattering (DLS). 3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay, Cell Counting Kit-8, Transwell assay, and flow cytometry were used to determine the effects of Conps on the biological function of BC cells. Western blotting was used to determine the changes in apoptosis-related proteins. Using FESEM and FETEM based on DLS and zeta potential determination, Conps was successfully constructed. Conps significantly lowered the viability of BC cells in a dose-dependent manner, significantly inhibited cell proliferation, invasion, and migration activities, and induced cell apoptosis. Western blotting demonstrated that Conps up-regulated Bax and caspase-3 proteins and down-regulated Bcl-2 protein. The results suggest that Conps can suppress the proliferation, invasion, and migration of BC cells and accelerate their apoptosis. This is a new potential strategy for the clinical treatment of BC.
In the context of "Internet Plus", how to teach English reading is very important. In this era of Internet development, how to use "Internet Plus" in junior middle school English reading teaching is also very important. This paper takes the students of No.11 middle school of Zunyi city as the research objects to conduct a questionnaire survey. After the survey and data analysis, it concludes that there are many problems in English reading teaching in the middle school. One is to ignore students' subjectivity; Second, the teaching mode is simple and traditional; Third, students' poor reading ability and lack of reading enthusiasm; Fourth, examination-oriented English reading teaching ignores the cultivation of students' reading ability. Finally, according to the existing problems, the application of "Internet Plus" in English reading teaching provides enlightenment and suggestions for junior middle school English reading teaching. First, access to useful reading resources through "Internet plus"; second, to conduct situational interactive learning through "Internet plus"; third, the integration of reading resources; Fourth, make use of the "Internet plus" new reading teaching mode; fifth, cultivate students' reading interest through "Internet plus"; finally, teachers carry out task-based reading teaching effectively. This paper has certain research significance, and the proposed solutions can be used to guide junior middle school English reading teaching.
