Lamina osteotomy and replantation with miniplate fixation is often used to treat benign intradural tumors, effectively preventing nerve entrapment and postoperative spinal deformities. However, no studies report using this technique to treat thoracic myelopathy due to ossification of the ligamentum flavum (OLF). This article reports the clinical outcome of a series of 18 cases of contiguous multilevel OLF treated by lamina osteotomy and replantation with miniplate fixation.Eighteen consecutive patients at the authors' institution were treated between 2008 and 2010 for contiguous multilevel OLF. Clinical efficacy, operative time, blood loss, sagittal alignment, and complications were investigated. Japanese Orthopaedic Association scale scores improved from 4.7±1.4 preoperatively to 7.9±1.3 three months postoperatively and 8.8±1.3 at final follow-up (P<.01), with a mean recovery rate of 67.8%±13.1%. No significant kyphotic deformity occurred postoperatively, and local kyphosis in the treated area increased by a mean of only 1.9°±1.0° at final follow-up. No patient required additional surgery due to spinal canal reobstruction and progressive spinal deformity. Cerebrospinal fluid leakage occurred in 4 patients and resolved after repair. Pulmonary infection and deep venous thrombosis occurred in 1 patient who was discharged with no complications after routine treatment.Lamina osteotomy and replantation with miniplate fixation is an effective therapeutic option for thoracic myelopathy due to contiguous multilevel OLF compression. The technique provides adequate decompression and stabilized sagittal alignment and avoids invasion of the spinal canal by scar tissue.
This paper has carried out soaking load test, routine soil test, X diffraction, scanning electron microscopy on coarse-grained soil in Gobi region, analyzed the coarse-grained soil collapsibility and explored the mechanism of collapsibility generation. The results showed that: the coarse-grained soil in Gobi region had a high degree of collapsibility; mineral composition, microstructure and salinity constituted the main internal reasons resulting in such phenomenon. In soaking conditions, the cementation strengthen between grains reduces, the structure is damaged under external force and the grain movement leads to greater settlement.
The principles of the continuous test methods for compaction degree is introduced based on subgrade engineering cases. The compaction parameter CMV associated with subgrade compaction is obtained through spectrum analysis on the response of a vibratory drum during compaction operations. Comparative analysis is conducted on CMV and the in situ test index Evd, which verifies that the continuous test index CMV has a high reliability and a good correlation with Evd and can objectively and comprehensively reflect the subgrade compaction quality and provides necessary basis for the application of the continuous quality test methods for subgrade.
Prestressed concrete(PC) channel bridge, consisting of a PC deck slab, two PC girders and two end crossbeams, is a new type of bridge structures. The PC channel bridges have been widely used in the structures such as highway, railway as well as urban rail transport. This paper presents a finite element method (FEM) analysis of mechanical behaviours of channel bridges with three girders in the construction, service and ultimate limit stage. The primary results include:(1) the channel bridges act elastically in the construction stage. (2) under the service loads, the deflections in the mid-span and the stresses on the whole concrete cross section could meet the requirements of the current codes. (3) the FEA results of flexural capacity of the channel bridges agree well with the results calculated according to the current codes, and the average loads of flexural capacity are about 3.26 times the design loads.
the measured data of subgrade settlement on Double Line 2 of Lanzhou-Xinjiang Railway are used to analyze the suitability of the hyperbolic method, Hushino method and Asaoka method to Gobi soil subgrade settlement forecasting. The sensitivity of the methods to the data fluctuation and the influence of the forecasting error and the selection of forecasting start time on the forecasting effectiveness were studied to determine the appropriate forecasting methods and forecasting time.
