Aim. To study the prevalence of arterial hypertension (AH) in the population of Kyrgyz small towns and countryside and to analyse ethnic features of AH.Material and methods. This cross-sectional epidemiologic study was performed as a part of the international study INTEREPID. A random sample of small town and countryside residents included 1774 people. The examination included a questionnaire survey, measurement of blood pressure, height, weight, and waist circumference, and laboratory tests. AH prevalence was age-standardised.Results. In Kyrgyz small town and countryside residents aged 18–65 years, the prevalence of AH was 44% (33,2% in men and 46,7% in women). Among Kyrgyz participants, AH prevalence was significantly lower than in their Russian peers (36,8% vs. 46,2%, respectively). This difference was observed in women, while men did not demonstrate any marked ethnic differences in AH prevalence.Conclusion. The results obtained suggest that AH prevalence in the Kyrgyz Republic is high (with a 40–50% increase over the past 20 years) and is characterised by marked gender and ethnic differences. Further research is required to identify whether race/ethnic group could be a risk factor for AH development.

Recent decades, cardiovascular diseases (CVD) remain the leading cause of mortality throughout the world. The prevalence of the main risk factors (RF) for CVD varies ethnically and geographically. Single observations witness that native and non­native populations inhabiting various, often quite contrast climatic and geographical, as socialeconomic conditions, might show a broad variety in CVD and RF prevalence.Aim. To evaluate the 4­year survival rate in association with the RF and cardiovascular morbidity and mortality among rural inhabitants of Russia and Kyrgyz Republic, with analysis of ethnic specifics (by the data from trial “Interepid”).Material and methods. The study was conducted under the framework of international project “Interepid” that included momentary epidemiological study of the prevalence of the main chronic non­communicable diseases and RF, and prospective step in small town and rural settlements of two countries — Russia and Kyrgyz Republic. In the analysis the results included of representative selections of rural inhabitants of the Volzhskiy region of Samarskaya Oblast of Russia (n=1050), and of Chuyskaya Oblast of Kyrgyz Republic (n=1341), age 20­64 y. o. In Russia and in Kyrgyz Republic the study was done by one protocol Interepid with specifically developed surveys (2011­2012). To the analysis, the endpoints included: all­cause mortality, CVD mortality, non­fatal cardiovascular events: non­fatal myocardial infarction, non­fatal stroke or transient ischemia, unstable angina, revascularization of any vascular pool — coronary bypass, endovascular revascularization, carotid endarterectomy, lower limbs arteries revascularization.Results. The standardised by age survival rate was higher among the Kyrgyz Republic inhabitants — 98,8%, and in Samarskaya oblast — 95,7%. In Kyrgyz Republic the fatal and non­fatal cardiovascular endpoints were registered in 14,7% responders, with no ethnical differences, and in Samarskaya oblast — only in 2,1% (p<0,01). Smoking had significant influence on adverse outcomes only in males in both selections. The influence of arterial hypertension was non­significant for the outcomes in Samarskaya Oblast during 4­year observation, however there was significant influence of this RF in Kyrgyz Republic. Overconsumption of animal fat among the Kyrgyz Republic inhabitants associated with the development of non­fatal cardiovascular outcomes, with no ethnical differences.Conclusion. The comparison of the results in three groups of population showed different impact of RF on development of the analyzed adverse outcomes in 4 years of prospective follow­up. Regardless the not that long term follow­up, the data obtained at the moment witness on reliable and prognostically significantly adverse influence of smoking, arterial hypertension and overconsumption of animal fat on the development of analyzed outcomes among the studied populations with ethnical and local differences. All these point the necessity is clear for development of differential management programs and events in Russian population and in native inhabitants of Kyrgyz Republic, including the specifics of CVD RF prevalence and influence on life prognosis.

Ельцина, Бишкек, Кыргызская Республика Цель исследования-изучение распространенности избыточной массы тела и ожирения среди сельских жителей Кыргызской Республики с анализом гендерных и этнических особенностей. Материал и методы. Данное исследование было проведено в рамках международного проекта «Интерэпид». Обследована представительная выборка населения северного региона Кыргызской Республики (n=1330) с откликом более 75%. Обследование включало опрос по специальной карте, состоящий из 9 модулей на основе адаптированных международных методик, объективные измерения (артериальное давление, рост, масса тела, пульс) и лабораторные анализы (липидный спектр, сахар, креатинин). Для оценки распространенности факторов риска использованы стандартные эпидемиологические методы. Для оценки избыточной массы тела рассчитывали индекс Кетле: масса тела (кг)/рост (м) 2. Наличие избыточной массы тела признавали при индексе массы тела (ИМТ) более 25 кг/м 2 , ожирения-при ИМТ 30 кг/м 2 и более. Абдоминальное ожирение диагностировали при окружности талии более 94 см у мужчин и более 80 см у женщин. Результаты. Распространенность ожирения в целом по группе составила 25,7% с преобладанием частоты его встречаемости у женщин (33,2% против 15,7%; р<0,001). Распространенность абдоминального ожирения составила 52,3%, при этом его частота также оказалась существенно выше у женщин по сравнению с мужчинами (68,2 и 31,0% соответственно; р<0,001). Выявлена линейная зависимость между частотой ожирения и возрастом как среди мужчин, так и женщин. Продемонстрированы этнические особенности в распространенности ожирения среди жителей республики. Показано, что кыргызы в целом существенно реже страдали ожирением, в том числе абдоминальным, чем русские респонденты (23,9% против 30,6%; p<0,01), что было характерно как для мужчин, так и женщин. Установлено, что ожирение наиболее тесно ассоциировано с наличием артериальной гипертензии (АГ), низким или средним уровнем образования, гиподинамией, а также такими компонентами метаболического синдрома, как гипертриглицеридемия и низкий уровень холестерина липопротеидов высокой плотности. Заключение. В Кыргызской Республике отмечается высокая распространенность ожирения, с преобладанием частоты его встречаемости среди женщин и русских респондентов. Наличие ожирения наиболее тесно ассоциировано с наличием АГ, низким или средним уровнем образования и гиподинамией. Существующее положение дел требует комплексного подхода, основой которого должна быть государственная политика по контролю популяционных значений массы тела, начиная с детского возраста, с вовлечением средств массовой информации, региональных властей и производителей пищевой промышленности.
