Морфологическая оценка состояния тканей пульпы зуба после смоделированной острой травмы под влиянием разработанной стоматологической лечебной прокладкой и трех аналогов при лечении глубокого кариеса и острого очагового пульпита. Материалы и методы. В эксперименте исследование проводилось на белых лабораторных крысах. Полость на жевательной поверхности моляра верхней челюсти формировали шаровидным бором. Сформированные экспериментально поражения поверхностного слоя пульпы покрывались с применением разработанного стоматологического препарата и трех аналогов. Экспериментальных животных выводили из эксперимента на 3, 7 и 30-е сутки. Зубы животных фиксировались в 10% нейтральном растворе формалина, декальцинировали 3 суток, затем заливали в гистологическую среду. Срезы получали толщиной 5-15 мкм. Результаты. В ходе экспериментального исследования была обнаружена активизация реактивных и компенсаторных процессов в ткани пульпы зуба с сохранением ее жизнеспособности при использовании разработанного стоматологического лечебного препарата и препарата сравнения Биодентин на 30-й день эксперимента. Гистологически данные изменения выражались в повышении уровня обменных процессов и увеличении активации клеточных элементов ткани пульпы зуба на 30-й день эксперимента. Заключение. В результате проведенного экспериментального исследования разработанного стоматологического лечебного препарата для лечения глубокого кариеса и острого очагового пульпита было установлено специфическое и многофункциональное свойство в предупреждении, профилактики развития воспаления и усиления защитных свойств ткани пульпы под воздействием данного препарата. Морфологическая оценка применения разработанной стоматологической лечебной прокладки продемонстрировала быстрое купирование воспалительной реакции ткани пульпы в сроки -30 суток и нормализацию основных компонентов пульпы зуба. Разработанный стоматологический лечебный препарат в ходе проведенного экспериментального исследования подтвердил заявленные качества, а именно: противовоспалительные, антисептические, обезболивающие и не уступает по своим свойствам представленным в ходе экспериментального исследования препаратам -аналогам. The article discusses the evaluation of the effectiveness of the influence of thermal vibration effects on the bending strength and surface hardness of composite filling materials.The study was conducted on the basis of the Department of Therapeutic Dentistry of the Kuban State Medical University, as well as at the Krasnodar Instrument Plant “Cascade”. The samples under study were made according to the group affiliation from three different composite materials: Estelite Sigma Quick, Filtek Bulk Fill Posterior Restorative and DentLight. The bending strength was evaluated on the “MIP-10” testing machine of the company “NPK TECHMASH”. Determination of the surface hardness according to Vickers was carried out on the device PMT-3 of the company “LOMO”.According to laboratory studies, a statistically significant increase in flexural strength and surface hardness according to Vickers was determined for all samples of composite materials subjected to thermal vibration before polymerization.The data obtained indicate an increase in the strength characteristics of composite filling materials as a result of thermal vibration exposure before their polymerization, which increases the service life of composite fillings and reduces the risks of secondary caries formation after filling.
Aim. Analysis of complications in the treatment of complicated caries.Materials and methods. Cone beam computed tomography results of 200 patients aged 21 to 64 years. The teeth were divided into frontal and chewing groups. The quality of the performed endodontic treatment was assessed.Results. During the analysis of the results of cone-beam computed tomography, the following complications of endodontic treatment were revealed: incomplete obturation of the root canal, removal of material beyond the apex of the tooth root, missed root canal, fracture of the instrument in the root canal. The regularity of their frequency was established, depending on the part of the jaw in which the tooth was located.Conclusions. Conducting cone-beam computed tomography during diagnosis, during treatment, will eliminate complicationsassociated with a lack of diagnostic imaging, effectively assess the success of the treatment for timely action.
Aim. This study was designed to evaluate the effectiveness of preventing the formation of a layer inhibited by oxygen or reducing its amount using various means.Materials and methods. The study was performed at the Department of Therapeutic Dentistry of the FSBEI HE KubSMU of the Ministry of Health care of Russia and the laboratory instrument factory "Cascade", Krasnodar. The composite Filtek Ultimate (3M ESPE, USA) was used for the production of model samples by placing the composite material in clean plastic molds with a diameter of 13.5 mm, thickness of 4 mm and polymerization, the following materials were selected to prevent contact of oxygen with the surface of the composite: glycerin solution, lavsan plate, Teflon tape. For the comparison with the standard method of elimination of the layer inhibited by oxygen we used the finishing of the surface of the composite. The surface hardness of the samples of each group and subgroup was measured using the apparatus-hardness tester PMT-3 by the Vickers method. The multivariate analysis of variance (ANOVA) was conducted in Statistica software 13. The samples were stained separately with a solution of "Lipton" tea from 2015 for 3 years. The degree of pigment deposition was evaluated.Results. The determining factor in the long-term functioning of the composite restoration is the stability of the surface layer. The results of the study showed the lowest microhardness of the samples from the group without any blockers of oxygen penetration without finishing (56HV), the highest microhardness was in samples from the group covered with Teflon tape with finishing (107HV). The probability value p<0.05 was obtained for all study groups. The results of the color change of the restoration were evaluated in the coloring medium. The assessment of changes in the coloring was carried out for 3 years. The results of staining allowed to confirm the obtained data of the study: 1) the intensity of staining of all samples increases over time, 2) the samples from the groups without glycerin coating were stained to a greater extent before the polymerization, without finishing in comparison with the samples from the groups without glycerin but with finishing, 3) the sample with glycerin coating and finishing was stained the least.Conclusion. Thus, the results of the study allow us to recommend using the means preventing the formation of the oxygeninhibited later to improve the efficiency of the restoration of the composite and increase the duration of its functioning. Depending on the clinical situation, glycerin, lavsan plate or Teflon tape can be used as oxygen blockers.
