BackgroundTreatment of rheumatoid arthritis (RA) prior to “Treat to target” is difficult enough, forcing researchers around the world look for ways to improve the effectiveness of RA treatment.ObjectivesTo explore the possibilities of reducing RA activity on the disease activity score DAS28 CRP by adding pentoxifylline to the methotrexate treatment.MethodsThis study included women (n=101) with RA longer than 1 year in duration, have a seropositive rheumatoid factor, and DAS28 CRP activity score of 3.2–5.1. Middle aged – 45.04±5.24 years old. All patients received a 15.50±2.50 mg oral dose of methotrexate per week. Patients were divided into two groups - those who only had methotrexate (n=50) and those who were treated with an oral dose of methotrexate and 1200 mg of pentoxifylline per day (n=51).ResultsThe baseline for both groups of RA patients did not differ significantly in terms of the level on the DAS28 CRP (p=0.812). On 14th day of the group who were taking pentoxifylline, the DAS28 CRP disease activity score was significantly lower by 12.5% (p<0.001). After 28 days, the users had a DAS28 CRP disease activity score index difference. The difference between two groups is statistically significant. In the group that were treated with pentoxifylline in addition to methotrexate (p=0.001) the index was found to be 8.3% lower. For 28 days, the methotrexate group's disease activity according to the DAS28 CRP activity score significantly decreased by 14.3% from the baseline (p<0.001). Personality traits of patients with pathology of hepatobiliary system (acute and chronic viral hepatitides, liver cirrhosis), their interrelationship with psychoemotional status (the state of personal and situational anxiety), formation of anxiogenic stress and the state of biomembranes have been studied. A conjugacy of a set of personality traits of patients with viral hepatitidis with increasing of personal and situational anxiety and formation of stress-induced disorder of the structural and functional state of biomembranes has been shown, that may be the base of the negative progression of viral hepatitides with forming cirrhosis of the liver. There is made a conclusion about worthwhileness of supplement the standard for diagnosis and treatment of patients suffered from viral hepatitides with psychometric methods, psychocorrective measures and preparations with antioxidant and membrane protection action.
ОРИгИНаЛьНЫЕ ИССЛЕДОВаНИя В ведение. В России общее количество больных ревматоидным артритом (РА) по эпидемиологическим данным составляет немногим более 1 млн человек (почти 1% населения) [1]. В течение первых двух лет приблизительно у 70% больных РА появляются рентгенологические признаки разрушения суставных тканей. Через пять лет от начала заболевания половина пациентов становится инвалидами и утрачивает работоспособность. Деструкция суставов, вовлечение в процесс других органов и систем, нежелательные побочные эффекты проводимой терапии приводят к выраженным психоэмоциональным нарушениям, преимущественно тревожно-депрессивного спектра и социальным ограничениям, что значительно ухудшает качество жизни (КЖ) пациентов [2]. По современным представлениям, РА, ТДР и ССЗ имеют не только общий провоцирующий фактор-хрони ческий психосоциальный стресс, но и ряд общих патогенетических механизмов. Ключевая роль принадлежит нарушениям иммунной систе мы провоспалительного характера [Т-лимфоциты, цитокины, интерлейкины (ИЛ)-1,-6,-8, фактор некроза опухоли (ФНО-a)], выявляемым при РА, психоэмо циональных нарушениях при РА. Системному воспалению с повышением ИЛ-6, ИЛ-8, ФНО-a отводится большое значение в формировании гипертонической болезни и развитии атеросклероза [3, 4, 5]. Цель исследования-оценить КЖ больных РА и корреляционных взаимосвязей его проявлений с выраженностью тревожно-депрессивных рас стройств при ассоциации с артериальной гипертензией. Материал и методы.
