We present a clinical case of urolithiasis. A patient diagnosed with stage 5 chronic kidney disease due to autosomal dominant polycystic kidney disease after bilateral nephrectomy underwent kidney transplantation with ureteral graft stenting. Two months after the operation, a stone was found in the upper third of the ureteral graft, complicated by necrosis in this area. Reconstructive plastic surgery on the ureter of the transplanted kidney with removal of the ureteral stone achieved the desired clinical effect.
Detection of carbapenem resistance genes is a critical issue for hospitals due to possible recommendations for infection control and targeted therapy. The Cepheid Xpert instrument, a Carba-R test for the detection and differentiation of five common carbapenemase genes, was tested from September 2020 to February 2021. As part of the approbation, 20 tests were provided. This review presents the results of the approbation of a relatively regular sensitivity study on Siemens WalkAway‑96 plus. Cepheid Xpert Carba-R analysis has been shown to be an accurate and fast tool for detecting colonization by carbapenem-resistant gram-negative bacteria, which can help limit the spread of these organisms in hospitals.
The concept of quality assurance in the health care system is popular for medical institutions all over the world. The problem of global multidrug resistance of pathogenic and opportunistic flora causes an urgent need for new approaches and methods of renal failure (particularly bacteriuria) rapid diagnosis.Objectives. Evaluate the benefits of laser light-scattering method in early diagnosis of renal failure and infectious complications after kidney transplantation.Methods. 220 urine samples from 129 patients were examined. Each sample was evaluated by traditional method and an automated laser light-scattering system simultaneously.Results. The automated method allowed to get the result in an average of 2 hours and give a result of bacteriuria availability with indicating the class of the pathogen by Gram. Discrepancy between the results of classical and automated methods was observed in 6.8 % of all samples, which is the standard for similar clinical trials.Conclusions. Laser light-scattering method has clear advantages over traditional methods. Diagnosis of urinary tract infections in kidney recipients in a shorter time (3.5 hours) is especially important. The economic benefit of automated process allows to reduce the costs of the hospital for bacteriological studies by 1.5 times.
