OBJECTIVE. This work investigated an influence of high-temperature oscillation plasma flow on hypoderma and determined the degree of influence on lymphatic vessels. MATERIAL AND METHODS. The experiment was carried out on 15 guinea pigs, that were divided into 5 groups (3 species in each group). An incision of derma and hypoderma was performed in dorsal area. The control group was the first and their incised tissues were used for comparative morphological analysis. Hypoderma has been treated by high-temperature oscillation plasma flow using Plasma.Jet to achieve coagulation homeostasis in the second group. Excisional biopsy was made immediately after tissue processing by Plasma.Jet in this group. Biopsy was performed on the third day in the third group. It was on the fifth day in the fourth group and on the seventh day in the fifth group. RESULTS. According to histological and immunohistochemical study, it was stated that optimal hemostasis was achieved due to coagulation without necrotic changes and absence of lymphorrhea. CONCLUSIONS. The developed method of hemostasis allowed doctors to reduce complication rate in postoperative period.
Importance of the topic. Using a net explant in Hernioplasty is one of the most common operations performed in surgical hospitals around the world. Despite of the achievement in chemical industry and the abundance of various endoprosthesis, there are always some drawbacks. Also, there is a problem in finding a plastic material that meets all the requirements of an ideal endoprosthesis and it is still relevant.Purpose. Study the timing of connective tissue germination of mesh implants made by different manufacturers in intact and compromised conditions (aseptic inflammation).Materials and methods. For the experiment we took 4 rabbits «Chinchilla» breed. We made 4 cuts on the back side of all animals (on both sides of the spine) which we used for the implantation of mesh implants. On the left side of the spine, the conditions were intact, on the right, aseptic inflammation was modeled by applying 0.3 ml of turpentine on liquid paraffin. On the 11th, 14th, 17th, 21st day an excision biopsy was performed. A complex of tissues was sent to histological examination, which would help us to rate the maturity of formed connective tissue that was evaluated by the severity of the inflammatory response.Results. In the end of our experiment we found out that the formation of the mature connective tissue around all endoprosthesis under conditions of aseptic inflammation occurred faster than in intact conditions. On the other hand, the time of germination in the explants was different, which should be considered in the postoperative period.Conclusion. None of the presented grids can be considered an ideal endoprosthesis.
The analysis of results of treatment of 113 patients with postoperative ventral hernias was carried out patients divided into two groups. 74 patients have received treatment (II (control) group) from 2013 to 2014 year. Patients of the control group were examined according to the accepted diagnostic algorithms, in addition to physical examination algorithm include chest and abdominal x-ray examination, ultrasound diagnosis of abdominal organs and fibro-esophagogastroduodenoscopy. Plastic by reticular endoprostheses was carried out in various methods. Methods have been chosen directly by surgeon during the operation. The draining of the postoperative wound was mandatory. Main (I) group included 39 patients who received treatment from 2015 to 2016. The diagnostic algorithm was supplemented with fibrocolonoscopy and spiral computed tomography, and the plastic method was predominantly sub-lay. Hypoderma treatment by high-temperature oscillating plasma flow energy according to the original technique for the prevention of lymphorrhea was provided to all patients. Maims were mostly sutured tightly. This approach allowed to reliably reduce frequency complications from 13.4% (14) to 1.8% (2) and, as a consequence, the length of stay of the patient in the hospital.
