The article presents current data on synovitis (inflammation of the synovial membrane), which is often the only clinical manifestation of osteoarthritis (OA), and considered to be one of the risk factors for the OA development. Synovitis severity correlates with the severity of OA symptoms. The assessment of the prevalence of synovitis presents significant difficulties and depends on the nature and severity of the disease, the cohort in question, diagnostic method, and therapy. The classification based on etiological, clinical and histological characteristics of synovitis is presented. Examination of synovial tissue characteristics and its changes during inflammatory response can significantly expand the possibilities for therapy and prognosis. The use of symptomatic delayed-action drugs (SYSADOA) is considered as the basic treatment of OA. Chondroitin sulfate (Chondroquard®) is one of the well established drugs, it has symptom- and structural-modifying properties. The article presents the results of studies confirming its effectiveness in reducing the severity of knee synovitis.
The paper gives the current definition of osteoarthritis (OA), which reflects the pathogenetic and clinical characteristics of this disease, as well as general principles for choosing an OA treatment. It describes the effect of glucosamine and chondroitin on the key pathogenetic mechanisms of OA. It is noted that one of the promising areas of therapy for OA is the intra-articular administration of biopolymer-based hydrogels that provide not only an anti-inflammatory, but also regenerative effect that has been experimentally confirmed during their injection into the tendon sheaths. There are data on the efficacy and safety of the Russian drug Sphero®gel, a biopolymer-based microheterogeneous collagen-containing hydrogel that belongs to a class of multicomponent biopolymer-based extracellular matrix mimetics. It consists of the cross-linked farm animal tissue-derived collagen microparticles placed in the gel base. The gel is not only a structural base for collagen microparticles; it also has its own therapeutic potential, since it is structurally similar to the natural extracellular matrix. The drug contains collagen, biologically active components of the extracellular matrix, such as proteoglycans, glycoproteins, uronic acids, growth factors, monosaccharides, and chondroitin sulfate. Extended-release symptomatic agents, Sphero®gel among them, are currently recommended for the treatment of OA. Application of Sphero®gel contributes to increased joint mobility and reduced pain, which allows the limited use of nonsteroidal anti-inflammatory drugs that cause adverse reactions, especially in the presence of comorbid diseases.
The study investigates the rule of spelling the root -ravn-/-rovn- and is considered to be a fragment of the academic description of Russian spelling, which is currently being under investigation at the Russian Language Institute of the Russian Academy of Sciences. The authors clarify the meanings that determine the spelling of the unstressed root, supplement the lists of exceptions, denote words with meanings not corresponding to the given values-criteria, and, for the first time in linguistics, investigate the words that can be correlated with different values-criteria, that is, they have double motivation. The rule codifies the spelling of words that have double motivation and fluctuate in usus, dictionaries, study guides and reference books. Spelling recommendations for these words correspond to the current linguistic norm and were approved by the Spelling Commission of the Russian Academy of Sciences in 2019. The linguistic commentary to the rule contains the most significant etymological facts concerning the root -ravn-/-rovn- and summarises the scientific and methodological attempts to figure out the distribution of vocabulary with root -ravn-/-rovn- based on the meanings selected in the spelling rules. In the paper it is shown that the instability in spelling of various verbs with the root -ravn-/-rovn- in modern writing and dictionaries is determined by the double motivation of words, as well as contradictory recommendations and gaps in the rules.
