Hypopituitarism is often developed in the result of operative treatment of tumours in chiasmal-cellular area. One of frequent indications of hypophysis’ deficit is decreasing of product of thyroid-stimulating hormone (TSH) that leads to deficit of thyroid hormones. Most patients, who had been operated in terms of tumours of chiasmal-cellular area, have normal volume and structure of thyroid gland in the background of decreased level of free thyroxine and absence of increasing of level of TSH on the principle of feedback, it indicates to the secondary character of destruction of thyroid gland. Patients operated in terms of craniopharyngioma and somatoprolactinoma more often have secondary hypothyroidism. A high per cent of fibrotic changes of tissue of thyroid gland in patients operated in terms of prolactinoma is explain by autoimmune thyroiditis in anamnesis.
Adenomas of pituitary (AP) make up about 10% of all intracranial tumours, which are met in practice of an endocrinologist or neurologist. As a rule AP are sporadically, however one must not consider them as an isolated disease. 52–62% of patients with hyperprolactinemia have AP. According to the data of. Federal Several of state statistics of the Russian Federation (2006) the amount of sick people with hyperprolactinemia among adult population can very from 214 thousand to 2 million people. About 80% make up young women at the age of 24–40 years old. 25% AP do not show superfluous hormonal secretion, they are clinically hormonally inactive, that embarrasses early diagnosis. In Novokuznetsk more than 50% of volumetrically brain formations make up AP. For last 10 years primary diseases AP has increased in 1.6 times. In Novokuznetsk there are factors increasing risk of forming of different neuroendocrinal diseases, high quantities of individual and population risks of oncologist pathology, deterioration of reproductive health, high levels of invalidism of population at able-towork age because of environment pollution and harmful work conditions. The examinations of expansion of AP disease at citizens of town Novokuznetsk show the necessity of introduction of register of dispensary for this category of sick people.
We have carried out a comparative analysis of peculiarities of headache at patients with adenoma of hypophis and hypotolamic syndrome of complicated genesis. 611 sick people were checked up (472 females and 139 males): 311 with adenomas of hypophis and 300 with hypotolamic syndrome. Methods of investigation: for appreciation of headache we analyzed qualitative and spatial — temporary descriptions and provokating factors. Double-sided headache at parients with adenomas of hypophis was in 2,5 more often than at patients with hypotolamic syndrome. Correlation of frequency of left- and right-side localization was also different: 1,6 : 1 — at sick people with hypotolamic syndrome and 1 : 1,1 — with adenomas of hypophis.
In 1992 Kemerovo region in the Russian Federation was first admitted as the area with an extraordinary ecological situation. Living in Kuzbass is factor of risk of forming of different endocrine diseases, deterioration of reproductive health, high quantities of individual and population risks of oncologist pathology, high levels of invalidism of population at able-to-work age because of environment pollution and harmful work conditions. The present study includes 299 sick people with confirmed diagnosis of adenomas of pituitary in the endocrine department of the town hospital № 1 for the period from 1995 to 2007 that is complete selection. Adenomas of pituitary are more often detected at people at active able- to-work age. Adenomas of pituitary are more often diagnosed at women 87,0%. The overwhelming majority of adenomas of pituitary (74,0%) is detected at the age of 20—50. Because of this disease 99 people (33,1%) have different degree of loss of their working ability. In the structure of adenomas prolactinomas dominate (53,5%), on the frequency of meeting hormonally inactive adenomas are on the second place (32,1%), somatotrophinomas are rarely met (in 11,7%) and the rarest ones at the population on Novokuznetsk are corticotrophinomas (in 2,7%). On dimensions of adenomas of pituitary microadenomas predominate — at 68,0% sick people. The relation microadenomas to macroadenomas is 17 : 22 at men and 187 : 73 at women.
