Midfacial fractures are presented by wide range of different patterns and severity. Orbital walls fractures have a special place in the structure of traumatic craniofacial injuries. Referring foreign authors, fractures of external rims and internal orbital walls are presented in more than 40 % of all bony injuries of facial skeleton. And referring Russian authors frequency of zygomaticomaxillary complex and isolated maxillary fractures presented by 12 and 8 % respectively. Furthermore complex anatomy of the orbit and proximity of the globe can occur some difficulties in diagnostics and treatment process of these patients. Today there are two main mechanisms of orbital walls fractures development – Blow-out, when fracture caused by increasing of intraorbital pressure, and Force-transmission, when force applied to external orbital rims transmits to a weaker orbital walls. Moreover in young patients Trapdoor mechanism is actual, which results in entrapment of soft tissues in the fracture line. Often there is an inferior rectus muscle entrapment and that is serious complication, when immediate surgery is required. The article deals with issues of anatomical construction of orbit and orbital trauma epidemiology based on statistics of neurosurgical department of Central city clinical hospital № 23, Yekaterinburg, for 2017.
Importance. Fractures of the orbital walls in structure of craniofacial traumatic injures occupy a special place and occur in more than 40 %. The complexity of the structure, the proximity of the vision organ location causes difficulties in diagnostics and treatment of these patients. Often surgical treatment is carried out with using of standard implants that must be manually and directly modeled during the surgical intervention. Such numerous manipulations with implant develop negative effect on strength, that reduces predictability of the results and causes various postoperative complications in 20 % of cases. The goal — is to conduct a systematic analysis of domestic and foreign literary sources to determine development prospects, improve diagnostic methods and treat orbital fractures. Methodology. A case-control or cohort study was reviewed with a very low risk of mixing effects and average probability of causal relationship. Conclusions. Surgical treatment of fractures of the facial middle zone, the orbital walls is a difficult task for surgeons. The great number of reconstructive materials and implants on the market provide a wide range of choices depending on the preferences of the surgeon and patient, that’s based on the specific clinical situation. However determining the ideal material for reconstructing of orbital walls is still an open question. Results. Digital and additive technologies have great perspectives in diagnostic and treatment of trauma of the face middle zone. However a number of tasks requires to introduce and adapt rapid prototyping technologies in practical healthcare, which will allow significant progress in the diagnostics and treatment of facial skeleton fractures.
АннотацияПредмет. Междисциплинарные исследования -основной тренд развития современной науки, в том числе и фун-даментальной медицины. В статье выполнен обзор оригинальных исследований авторов и современной литературы на стыке фундаментальной стоматологии и биоминералогии -в области материаловедения биоминеральных образо-ваний -зубных тканей человека.Цель. Обоснование нового междисциплинарного научного направления -минералогической стоматологии; формули-ровка предмета и методов исследования; подведение итогов работы и обоснование перспектив развития.Результаты. Рассмотрены результаты системных физико-химических исследований зубной ткани (витальной и при раз-личных патологиях), в том числе работ уральской школы в области материаловедения зубов с повышенной стираемостью (особенностей их фазового и химического состава, структуры, механических свойств и др.), механизма развития заболе-вания, прикладных вопросов использования результатов в стоматологической практике при реставрации зубов и др. Проа-нализированы работы зарубежных авторов, в особенности использующих локальные методы исследования зубных тканей и картирования их свойств. Рассмотрены вопросы картирования тканей, основанного на использовании цифровых данных различных локальных методов анализа вещества -сканирующей (просвечивающей) электронной микроскопии с EDX ана-лизом, микрозондового анализа, масс-спектрометрии с лазерной абляцией, ИК-и рамановской микроспектроскопии и др.Выводы. На стыке фундаментальной стоматологии и биоминералогии сформировалось новое междисциплинарное научное направление -минералогическая стоматология; в центре ее внимания стоят исследования твердых тканей зуба человека, реставрационных материалов, конструкций, зон взаимодействия (гибридизации) с использованием приемов и подходов, наработанных в биоминералогии и основанных на применении современных аналитических методик.Работа выполнена в рамках научной школы НШ-9723.2016.5.
