Background. Mediastinal malignant germ cell tumors are relatively rare and treatment for cisplatinresistant patients is not established. Case. A 25 year-old man was admitted to our hospital because of chest pain. Chest CT film showed a large tumor (10 x 9 cm) in the anterior mediastinum. Percutaneous needle biopsy of the tumor revealed seminoma. The chemotherapy with BEP and radiotherapy were ineffective. Therefore, high-dose chemotherapy with peripheral blood stem cell transplantation (PBSCT) was performed and tumor decreased. Subsequently, tumor was completely resected and there has been no sign of recurrence for one year. Conclusion. High-dose chemotherapy with PBSCT was effective treatment for cisplatin-resistant mediastinal malignant germ cell tumor.
