18-Fluorodeoxyglucose Positron Emission Tomography in the Early Diagnostic Workup of Differentiated Thyroid Cancer Patients with a Negative Post-Therapeutic Iodine Scan and Detectable Thyroglobulin

Abstract: An (18)F-FDG-PET within three months after a negative WBS with detectable Tg-off showed additional tumor localization in 17% of the patients, leading to a change in clinical management in 13%. A clinically useful Tg-off cutoff value was not found, but 90% of positive (18)F-FDG-PET scans occurred in patients with a Tg-off >2.00 ng/mL.

“…The literature search identified 34 studies with a total of 2639 patients meeting the inclusion criteria ( 6 , 16 48 ). Eleven studies reported on the diagnostic accuracy of single-modality PET, 17 on PET/CT, and six on both PET and PET/CT (“mixed”).…”

“…In 2013, Van Dijk et al [37] presented a study on 52 patients with DTC, Tg (+) during thyroxine withdrawal, who underwent FDG PET within three months after the negative posttherapeutic WBS. Additional tumor localization has been showed in 17% of patients, leading to a change in the clinical management in 13%.…”

“…All patients underwent a (near) total thyroidectomy and on indication a lymph node dissection, followed by Tg-off measurement and 131 I (1850-5550 MBq) treatment, according to our institutional protocol and the Dutch guidelines (http://www.on coline.nl/schildkliercarcinoom). 13 Initial curative treatment was completed if there was no evidence of residual disease, including no uptake on the diagnostic whole body scan (WBS) and a Tgoff measurement <1Á0 lg/l, 6 months after 131 I therapy. Patients received regular follow-up with Tg-on measurement, clinical examination and US.…”
