40% of Females with Mantle Cell Lymphoma Are Managed with Initial Observation: Results from the MCL Biobank Observational Study

Abstract: Background: Although typically an aggressive disease some patients with Mantle Cell Lymphoma (MCL) follow an indolent course and may not benefit from immediate therapy. Several studies evaluating watch-and-wait (W&W) strategies in MCL show no detriment to patient survival and international guidelines acknowledge the strategy's suitability for a minority of patients. Differences in pathogenetic mechanisms have provided insight into variable clinical behaviour, but there is little clinical evi… Show more

“…Most of these cases are characterised by a leukaemic, non-nodal lymphoma manifestation, a very low Ki-67 Index (<10%) or have measurable disease without markers of higher cell turnover (raised LDH and Ki-67 ≥30%). 72 In these cases, watchful waiting under close monitoring is considered an appropriate strategy, 73 with up to half of patients not requiring treatment in the first 2 years of observation according to data from the UK National Cancer Research Institute (NCRI) MCL Biobank observational study. 72 However, most newly diagnosed patients display aggressive disease features and require early initiation of therapy.…”

“…Despite its heterogeneous presentation ranging from indolent to aggressive, MCL is differentiated from all other lymphomas in demonstrating a male‐to‐female ratio of ≥2–2.5:1 across multiple studies with no clear underlying pathobiology for this significant imbalance 1 . The MCL Biobank Observational Study based out of the UK evaluated 315 patients with newly diagnosed MCL between 2015 and 2018, noting a male predominance of 68.9%, supporting this observation 2 . We therefore hypothesised that the androgen receptor (AR) signalling axis may be a driver of lymphomagenesis in MCL.…”
