A break-even analysis of delivering a memory clinic by videoconferencing

Abstract: We analysed the costs of two kinds of dementia clinic. In the conventional clinic, held in a rural area, the specialist travels to the clinic from the city. In the videoconferencing clinic, patients are also seen in a rural area, but the specialist conducts the assessment by video from the city. The fixed costs common to both modalities, such as clinic infrastructure, were ignored. The total fixed cost of a monthly conventional clinic was $522 and the total fixed cost of a monthly videoconferencing clinic was … Show more

“…A large cluster randomized trial would need to be undertaken to prove equivalence or superiority of this model for clinical outcomes. The current model can be justified in two ways: single‐provider dementia diagnosis with telehealth has been demonstrated to be reliable, and interprofessional geriatric assessment has been repeatedly proven to improve clinical outcomes.…”

“…As a VHA intervention, it has been shown to have multiple benefits, including enhanced veteran access to professionals, efficiency, and greater collaborative care. It is most effective when medical concerns are complex and when mental health care is provided, but uses of telehealth in the health care of individuals with dementia have typically been limited to single provider evaluation, follow‐up of established patients, psychiatric services, and models not including geriatricians as team members …”

“…Apart from the workforce, and the building cost, there is also the need for video conferencing equipment (a set at the local and remote site). Economic studies have shown that when a certain number of patients need to be seen, or the distance is extreme, regardless of the need for extra staff and extra equipment, it is cost effective to have a telehealth service [15]. Efficiencies established by maximising the equipment use improve the business case for telegeriatric memory disorder clinics.…”

“…A video conference consultation (VC) is a reliable alternative and can eliminate some of the unnecessary anxiety caused by travel and the necessity of adapting to an unfamiliar environment for the assessment process [7]- [13]. The success of such a service relies heavily on the acceptance of referring GPs, patients and their care givers [14], and the economic viability of such a service [15]. Despite the evidence, the uptake of telehealth memory clinic services has been slow.…”

“…Engage telehealth services to both educate providers and reach rural residents: Telehealth is a strategy that may overcome challenges such as reaching rural populations, improving patient and provider knowledge, and reversing negative clinician attitudes. 45,46 Using providers who are knowledgeable about AD and experienced in the respective culture is critical for the success of educational programs.
