A Case of Carpal Intersynovial Fistula in a Horse

Abstract: SUMMARY A tentative diagnosis of a fistula between the tendon sheaths of the right extensor carpi radialis and the intercarpal joint was confirmed by positive contrast radiography following injection of sodium diatrizoate into the joint. Surgical removal of a tag of synovial membrane and repair of the fistula was carried out. Post surgical treatment included mild exercise to prevent adhesions within the sheath. A month following discharge, contrast radiography revealed no communication between joint and tendon… Show more

“…Wheat (1978) is reported to have repaired a fistula between the extensor carpi radialis sheath and the intercarpal joint caused by distal radial fracture. The surgical repair of a fistula between the intercarpal and carpometacarpal joints, and the extensor carpi radialis sheath, was described by Llewellyn (1979) and he stressed that positive contrast radiography was essential to confirm the diagnosis of such lesions.…”

“…Magnetic resonance imaging evaluation of the carpal region demonstrates the close proximity between the antebrachiocarpal joint and extensor carpi radialis tendon (Kaser-Hotz et al 1994). Given the anatomic apposition between the carpal joints and forelimb extensor sheaths, iatrogenic (Johnson and Ryan 1975;Wilson 1989;Goodrich and McIlwraith 2009;McIlwraith et al 2014) and traumatic (Llewellyn 1979) communication between the structures can occur.…”

“…Plain and contrast radiography were essential to exclude conditions such as carpal ganglia and intersynovial fistulae (Llewellyn 1979). These conditions require surgical repair of the synovial membrane rather than drainage and pressure bandaging, and an infection arising from an open drain could prove disastrous if the swelling communicated with the joint.…”
