A Case of Fasciola hepatica Infection Mimicking Cholangiocarcinoma and ITS-1 Sequencing of the Worm

Abstract: Fascioliasis is a zoonotic infection caused by Fasciola hepatica or Fasciola gigantica. We report an 87-year-old Korean male patient with postprandial abdominal pain and discomfort due to F. hepatica infection who was diagnosed and managed by endoscopic retrograde cholangiopancreatography (ERCP) with extraction of 2 worms. At his first visit to the hospital, a gallbladder stone was suspected. CT and magnetic retrograde cholangiopancreatography (MRCP) showed an intraductal mass in the common bile duct (CBD) wit… Show more

“…The diagnosis of fascioliasis is often delayed due to non-specific radiological and clinical findings. Patients infected with F. hepatica are typically admitted with symptoms of biliary colic, liver abscess, choledocholithiasis, pancreatitis, and compressive symptoms mimicking cholangiocarcinoma (11,12,14). ERCP can be performed for the differential diagnosis and definitive treatment of F. hepatica, thus avoiding further surgery (16).…”

“…The utility of ERCP in the treatment of biliary system parasites differs according to parasite type and intervention timing. For example, parasite extraction provides a therapeutic option for F. hepatica if antimicrobial therapy fails to completely eradicate biliary system parasites (10)(11)(12)(13)(14). Moreover, ERCP provides a bridge to safe surgery for Echinococcus granulosus by extracting vesicles from the biliary channel preoperatively.…”

“…In Korea, cattle were heavily infected with Fasciola species before 2000 with a prevalence of up to 38.2% [ 7 , 8 , 9 ]. Additionally, there were several cases of human infections with F. hepatica [ 10 , 11 ]. However, recent studies on trace animals and the prevalence of fascioliasis are lacking.…”

“…Mammalian hosts acquire F. hepatica infection through ingestion of contaminated vegetation or water containing viable metacercariae of F. hepatica throughout their lifetimes [25,26], so older hosts have more opportunities to ingest encysted dormant larvae. In the present study, age was considered as a risk factor associated with F. hepatica infection in yaks.…”
