A case of lymphomatoid papulosis and systemic lymphoma*

Abstract: A young man with a 4-year history of lymphomatoid papulosis subsequently developed a systemic lymphoma. Further doubt is therefore cast on the benign nature of this disorder.

“…No difference has been found in the clinical picture and histology of LyP between patients who developed malignant lymphoma and those who did not. 11,18,19 Techniques such as T-cell receptor gene rearrangement studies, immunophenotyping or flow cytometry studies have not proven to be helpful in predicting which patients eventually develop a malignancy. 5,14,18 In childhood-onset LyP, malignant transformation has never been described, even if the disease carries on into adulthood.…”

“…However, even among patients with 'classical' lymphomatoid papulosis transformation to lymphoma may occur. Messenger, Marshall & Summerly (1981) postulated a possible viral role in the development of malignancy in lymphomatoid papulosis, finding a rise in their patient's Epstein Barr virus (EBV) titre after he had developed a lymphoma. EBV is implicated in the pathogenesis of Burkitt's lymphoma and nasopharyngeal carcinoma, and more recently another Herpes virus, cytomegalovirus (CMV) has been postulated to bear a causal relationship to the development of Kaposi's sarcoma in young male homosexuals and other patients affected by the acquired immunodeficiency syndrome (AIDS) recently described in the U.S.A. (Giraldo et ai, 1972(Giraldo et ai, , 1978.…”

“…Lymphomatoid papulosis has so far been considered a clinically benign disorder but with malignant histology (Thomsen, Hjort & Svendsen, 1972;Macaulay, 1978;Black, 1981;Weinman & Ackerman, 1981). Although the majority of reported cases had a benign course, the number of published cases that have developed systemic lympho-proliferative disease is increasing (Kawada, Anekoji & Miyamoto, 1969;Black & Wilson-Jones, 1972;Dupont, 1973;Messenger, Marshall & Summerly, 1981;Scheen, Doyle & Winkelmann, 1981). Lymphomatoid papulosis is easily diagnosed by the clinical criteria in combination with the rather characteristic histological changes (Weinman & Ackerman, 1981), but the histology may differ widely even in the same patient.…”
