A Case of Myocarditis and Near-Lethal Arrhythmia Associated With Interleukin-2 Therapy

Abstract: We present a case of a 48-year-old female who developed myocarditis and near fatal arrhythmias during high dose Il-2 therapy for metastatic renal cancer. On day 5 of therapy, the patient developed sudden onset chest pain, elevated cardiac enzymes and ST segment changes on EKG. Coronary angiogram was normal, however echocardiogram showed reduced ejection fraction and hemodynamic measurements showed elevated bilateral elevated filling pressures. The patient then developed episodes of recurrent ventricular arrhyt… Show more

“…IL-2 has also been associated with the recurrence of AF in patients undergoing catheter ablation (5). In addition, high-dose IL-2 therapy has been significantly associated with cardiac toxicities in patients with cancer, often resulting in lethal arrhythmias (6)(7)(8). However, the underlying mechanism of IL-2 in the pathogenesis of CAs still remains unclear.…”

“…70 Additionally, IL-2 has been associated with an upregulation of SCN3B expression and a resultant increase in sodium current density. 71 Myocarditis and near fatal arrhythmias during high dose IL-2 therapy for metastatic renal cancer were recently reported, 72 which reinforces the potential use of IL-2 levels as a marker for arrhythmias in Chagasic patients.…”
