Smooth muscle hamartoma (SMH) and striated muscle hamartoma (STH) are anomalous proliferations of smooth muscle or striated muscle, respectively, in anatomic sites where these tissues are normally present. To date, only limited cases have been reported describing these lesions. In this study, we sought to characterize the clinicopathologic features of both SMH and STH. A total of 27 cases of SMH and 12 cases of STH from 1990 to 2020 were identified. SMH cases had a slight male predominance (63%) and a mean age of presentation of 20 years (range: 4 months‐91 years), with a mean size of 9.3 mm (±13.3). In contrast, STH were equally distributed in gender, with a mean age of presentation of 40 years (range: 3 months‐66 years) and a mean size of 5.7 mm (±3.6). SMH were more commonly located in the torso and extremities (70%) and STH in the head and neck area (92%). One case of SMH recurred after 1.1 years and in the initial diagnosis the lesion was present at the tissue edge. None of the cases of STH had a recurrence. We present the largest cohort of SMH and STH, and report the first case of a recurrent SMH, suggesting the importance of obtaining a clean margin for these lesions.

“…Tanaka et al reported a case of multiple painless nodular swellings over left cheek in a 15-year-old boy with no significant change in size since birth which was then excised and histopathologically shown as striated muscle hamartoma. 15 Al Omran et al reported a case of painless right facial swelling in a five-year-old girl since two months which upon investigating revealed a well-defined ovoid mass within the superficial portion of the parotid gland in CECT which after excision was diagnosed as a vascular hamartoma. 16 For our patient, ultrasound and Doppler were done, which revealed a capillary hemangioma in the intramuscular compartment, and thus we didn't do an FNAC in view of its vascularity.…”

<p class="abstract">Hamartomas are non-neoplastic, tumor-like masses characterized by irregular overgrowth in tissues of its origin, resulting from inborn errors that occur during embryonic development and are present from birth. They may develop from any of the three germinal layers of which mesodermal-derived overgrowth is the most common. Hamartomas may be seen anywhere in the body and most often arises from lung, liver, spleen, kidney, gastrointestinal tract, and rarely in the head and neck region. A 23-year-old lady presented with a painless left cheek swelling for three months. On clinical examination, she had a single, firm and ill-defined swelling of size 2×1cms in left cheek. Ultrasound examination showed an ill-defined hypoechoic lesion measuring 2.1×0.8 cms in the intramuscular compartment of left cheek with few cystic areas. On colour doppler there was minimal peripheral vascularity and flow signal suggesting capillary hemangioma. In view of the tumor’s vascularity, Fine needle aspiration cytology wasn’t performed and she was treated conservatively on oral propranolol. But since no significant response was noted, excision biopsy was done under general anesthesia. The histopathology features were consistent with mesenchymal hamartoma. Post-operative period was uneventful, and she was in regular follow up with no recurrence.</p>

Cutaneous mesenchymal hamartoma with mixed myogenous differentiation

A 3-month-old infant girl presented with a polypoid lesion in the perianal region. No changes in this lesion had been noted since birth. Microscopic examination of the excised specimen showed a mixture of mesenchymal elements, dominated by haphazard thin fascicles of skeletal muscle. Collagen and vascular changes were also apparent. Immunohistochemistry showed positive staining for muscle-specific actin and desmin in the fascicular components of the lesion, and smooth muscle actin, desmin, and h-caldesmon positivity in a haphazard collection of muscle fibers in the deep dermis and anal submucosa. Numerous Verhoeff-van Gieson positive elastic fibers were also noted. Hamartomas containing skeletal muscle have rarely been reported outside of the head and neck region. They must be distinguished from a variety of other tumors, including juvenile rhabdomyoma, benign Triton tumor, and rhabdomyosarcoma.
