A case report of reactive solitary eccrine syringofibroadenoma

Abstract: Eccrine syringofibroadenoma is a very rare benign tumour of acrosyringium of eccrine sweat duct. Based on the evidences of known etiological factors, two forms have been proposed; reactive and nonreactive. Reactive forms are rarer, and on even rarer occasions, trauma complicated by secondary nonspecific infections may lead to the development of reactive eccrine syringofibroadenoma, as in our case. Here, we are documenting a case of reactive solitary eccrine syringofibroadenoma in a 65-year-old male presenting … Show more

“…Clinically, lesions could be solitary or multiple, pink, or skincolored coalescing nodules of variable sizes. 1 Histologically, ESFA should be differentiated from fibroepithelioma of Pinkus, hidroacanthoma simplex, tumor of the follicular infundibulum, poroma, reticulated seborrheic keratosis, and squamous cell carcinoma with ductal differentiation. 3,12,13 The association of reactive ESFA with a malignant skin condition is extremely rare.…”

“…Generally, it is slow-growing and asymptomatic for the patient, although some patients might experience itching or pain. Sometimes poromatosis is present, with multiple, widespread lesions at the moment of diagnosis [ 11 , 227 ]. In the dermatoscopic examination, a polymorphic vascular pattern is probably the most prominent finding facilitative in diagnosis.…”

“…Reactive ESFA in association with ulceration has also been reported with different etiologies such as diabetes mellitus, traumatic injury, venous insufficiency, burn scar, and infections such as leprosy. [2][3][4][5][6] In the review of the literature, approximately 40 cases of reactive ESFA have been described containing eight patients with a chronic leg ulcer. 4 In accordance with our case, Sirikham et al Reactive ESFA, especially its multiple forms, could cause both functional impairment and cosmetic impact on the patients' quality of life.…”

“…5 Other therapeutic options are ablative modalities such as CO 2 laser, and also radiotherapy in the sites hard to access. 3 All the physicians and dermatologists should be considered the diagnosis of reactive ESFA as one of the differential diagnoses in slow-growing lesions next to chronic ulcers. Regarding the possibility of malignant transformation of ESFA, complete excision must be contemplated as the gold standard treatment.…”
