Abstract:Objective: Many investigations have studied the associations between matrix metalloproteinase-9 (MMP-9) C1562T polymorphisms and coronary artery disease (CAD). However, the conclusions of these studies were inconsistent. Therefore, this study was aimed at clarifying the association between MMP-9 C1562T polymorphisms and CAD in a large-scale meta-analysis. Methods: The PubMed and Embase databases were retrieved to collect all publications on the association between MMP-9 C1562T polymorphisms and CAD. Then the odd ratios (ORs) and 95% confidence intervals (95% CIs) for C1562T TT+TC versus CC genotype between CAD and the control groups were evaluated. Subgroup analysis was also performed according to different races. The meta-analysis was performed by Stata 10.0. Results: Sixteen case-control studies were included in our meta-analysis, involving 11 032 CAD patients and 4628 non-CAD controls. Compared with C allele carriers, East Asian T allele carriers TT+TC had a significantly higher risk of CAD (OR=1.43; 95% CI: 1.03-1.99; P=0.031); however, there were no significant associations in Western populations (OR=1.06; 95% CI: 0.96-1.18; P=0.240) or West Asians (OR=1.13; 95% CI: 0.75-1.70; P=0.565). When further analyzing the association between C1562T polymorphisms and myocardial infarction (MI, the most serious type of CAD), the risk of TT+TC genotype versus CC genotype for MI was significantly higher for the overall (OR=1.21; 95% CI: 1.04-1.40; P=0.012) and for East Asians (OR=1.58; 95% CI: 1.26-1.97; P=0.000) but not in Western populations (OR=1.12; 95% CI: 0.99-1.26; P=0.078). Conclusions: Our meta-analysis suggested an obvious ethnic difference in the association between MMP-9 C1562T polymorphisms and CAD. MMP-9 C1562T polymorphism was significantly related to CAD in East Asians. However, no significant associations were observed in either West Asians or Western populations.

We examined whether two functional polymorphisms (g.À1562C4T and g.À90(CA)14-24) in the matrix metalloproteinase (MMP)-9 gene or MMP-9 haplotypes affect the circulating levels of pro-MMP-9 and pro-MMP-9/TIMP-1 (tissue inhibitor of metalloproteinase-1) ratios in AIDS patients, and modulate alterations in these biomarkers after highly active antiretroviral therapy (HAART). We studied 82 patients commencing HAART. Higher pro-MMP-9 concentrations and pro-MMP-9/TIMP-1 ratios were found in CT/TT patients compared with CC patients. HAART decreased pro-MMP-9 levels and pro-MMP-9/TIMP-1 ratios in CT/TT patients, it did not modify pro-MMP-9 levels and it increased pro-MMP-9/TIMP-1 ratios in CC patients. The g.À90(CA)14-24 polymorphism, however, produced no significant effects. Moreover, we found no significant differences in HAARTinduced changes in plasma pro-MMP-9, TIMP-1 and pro-MMP-9/TIMP-1 ratios when different MMP-9 haplotypes were compared. These findings suggest that the g.À1562C4T polymorphism affects pro-MMP-9 levels in patients with AIDS and modulates the alterations in pro-MMP-9 levels caused by HAART, thus possibly affecting the risk of cardiovascular complications.

ABSTRACT:The role of the 1562C>T single nucleotide polymorphism of the matrix metalloproteinase 9 (MMP-9) gene promoter in acute coronary syndrome (ACS) development has been reported in various populations. In this work, we investigated the association between MMP-9 1562C>T polymorphism and ACS patients in Vietnam. This study was conducted on 138 ACS patients and 68 control subjects recruited from the Vietnam National Heart Institute. The genotype of 1562C>T polymorphism was determined by RFLP-polymerase chain reaction and the serum MMP-9 level was measured by the ELISA method. We found that the frequencies of CT and TT genotypes in the ACS patients (30% and 4%) were higher than those in the control (18% and 1%). The 1562T allele in the MMP-9 promoter was found to have a significantly higher frequency in ACS patients than in control subjects (ACS versus control: 19% versus 10%, p = 0.001). Multiple logistic regression analysis indicated that the MMP-9 1562T allele carriers had an increased risk of developing ACS (odds ratio = 2.37; 95% confidence intervals: 1.04-5.75, p = 0.045). The serum MMP-9 level in the polymorphism-carrying group was considerably higher than in the group without polymorphism in both ACS patients (229 ± 83 versus 194 ± 108, p = 0.037) and controls (220 ± 41 versus 171 ± 80, p = 0.006). Our results showed that the MMP-9 1562C>T polymorphism is significantly associated with the ACS susceptibility in the Vietnamese population.
