A cluster of two human cases of tick-borne encephalitis (TBE) transmitted by unpasteurised goat milk and cheese in Germany, May 2016

Abstract: In May 2016, two cases of tick-borne encephalitis (TBE) were confirmed by serology (positive IgM and IgG antibodies against TBE virus (TBEV) in serum), with a possible link to raw milk and cheese from a goat farm in a region in Baden-Württemberg, Germany not previously known as TBE-endemic. The outbreak investigation identified 32 consumers of goat dairy products (29 consumers, one farm employee, two owners) of whom none had IgM antibodies against TBEV 3-8 weeks after consumption. Of the 27 notified TBE cases … Show more

“…If such a model has existed for a long time, it should have resulted in a thoroughly intermixed Eurasian TBEV population, where identification of the types would not have been possible, but this is not the case. This observation is compatible with a recent diversification of TBEV in general and with the current rapid European TBEV expansion to novel areas and habitats [20,39,40,[62][63][64][65]. Predicted past population dynamics (Figure 9) is also in concordance with this data.…”

“…Astrocytes enter a reactive stage (6) and some of them die (7). Both neurons and astrocytes overexpressed a high level of chemokines involved in chemo-attraction of T cells in the brain parenchyma (8), although astrocytes are stronger producers. Figure was created using Servier Medical Art available on www.servier.com.…”

“…TBEV is usually transmitted to humans from infected ticks, mainly of the Ixodes family, but may occasionally be acquired by consumption of unpasteurized dairy products from infected livestock [7][8][9]. Upon inoculation into the human skin, initial infection and replication occur in local dendritic cells (DCs).…”
