A Comparison of Free Autologous Breast Reconstruction with and without the Use of Laser-Assisted Indocyanine Green Angiography

Abstract: The authors' study demonstrates that laser-assisted indocyanine green angiography is a cost-effective technology under the most stringent acceptable thresholds when used in immediate free autologous breast reconstruction.

“…25,26 The ICG angiography has been used for global assessment of microvascular and macrovascular flow within tissues, and though reports of the utility of ICG angiography to visualize the periprosthetic capsule have yet to be published, the technique has been used clinically to predict mastectomy flap necrosis, limb ischemia, free flap monitoring, and other related areas. [25][26][27][28][29][30] The proposed methodology described herein can be readily expanded to include all of the major commercially available ADMs to provide a means to quantify and classify ADMs by these performance parameters to guide ADM selection by the reconstructive surgeon. Each ADM has a unique profile based on its tissue type and origin, tissue processing technique, and sterilization measures that accounts for their biologic performance.…”

“…97,98 Alternatively, the Spy Elite system (Novadaq Technologies, Mississauga, ON, CAN) is reported per use; depending on the financial arrangement, supply kits-not the unit itself-generate cost. [99][100][101] ICG is inexpensive, therefore marginal cost can be quite low. 97,102 Because wide variations exist in pricing and cost structure (Table 4), cost-utility analyses may be unreliable.…”

“…14 In recent years, several studies have been published validating the effectiveness of laser-assisted indocyanine green angiography in postmastectomy skin flap assessment. [15][16][17][18] However, it has not been directly compared to fluorescein with regard to its effectiveness in preventing wound healing complications. The purpose of this study was to compare three methods for assessing mastectomy flap viability following skin-sparing mastectomydirect inspection, fluorescein dye, and indocyanine green angiography-to determine which is the most effective in reducing complications related to mastectomy flap necrosis.…”
