A comparison of the effect of various topically active steroids on the clinical and histological features of psoriasis

Abstract: SUMMARY The effect of betamethasone 17‐valerate, fluclorolone acetonide, fluocinolone acetonide and formocortal on the clinical and histological features and mitotic counts of psoriatic epidermis have been compared. In five of the six patients studied the preparations were used under polythene occlusive dressings. Biopsies were carried out before treatment and then at varying intervals for a week. All the four steroid preparations appeared effectively to reverse the psoriatic lesion. Betamethasone 17‐valerate … Show more

“…Thus, retinoids block the formation of a normal stratum corneum and induce numerous extra vital cell layers in upper epidermis (Tammi and Tammi, 1986). Pharmacological doses of glucocorticoids, on the other hand, enhance normal keratinocyte differentiation and reduce the number of vital cell layers, as shown in cultured epidermis (Tammi and Tammi, 1986) and in the successful treatment of many skin diseases with disordered keratinization (Fry and McMinn, 1968;Goodwin et al, 1973). Glucocorticoids may even play a specific role in the induction of keratinization Endo, 1969, 1971;Obinata and Endo, 1977).…”

“…The bio-availabilities and activities of many proprietary topical corticosteroid formulations were assessed by the single-application occluded vasoconstrictor test by Barry & Woodford (1974, I975> 1976. That work permitted rank order classification of the preparations and often demonstrated comparability between the results obtained and those reported in clinical trials (Portnoy, 1972;Baran, 1972;Rex, 1973;Goodwin, Hamilton & Fry, 1973;MacDonald & Fry, 1974;Sparkes & Wilson, 1974;Whitefield & McKenzie, 1975;Corbett, 1976;Wilson, 1976). However, in the treatment of dermatological disease a steroid preparation is not usually applied once only under occlusion but may be rubbed into the non-occluded affected area, often several times a day.…”

“…Evidence for a basic defect in the skin derives from studies of epidermal proliferation in psoriatic plaques [23,24]. Transplantation and recombinant grafting studies in nude mice have implicated components of both the epidermis and dermis [25, 261.…”
