A Comprehensive Review of the Diagnosis and Management of Acute Liver Failure

Nazli Begum Ozturk,
Emre Herdan,
Fuat H. Saner
et al.

Abstract: Acute liver failure (ALF) is a rare and specific form of severe hepatic dysfunction characterized by coagulopathy and hepatic encephalopathy in a patient with no known liver disease. ALF carries a high morbidity and mortality. Careful attention should be given to hemodynamics and metabolic parameters along with the active surveillance of infections. Timely transfer and supportive management are important in an intensive care unit in a liver transplant center. Identifying patients who will and will not improve … Show more

“…In contrast to acute single ingestion, subacute or chronic toxicity emerges from the cumulative effect of multiple doses of acetaminophen over time, surpassing recommended limits [8]. With repeated use, there is an inherent risk of gradual metabolite accumulation, particularly the toxic NAPQI, within the liver [10].…”

“…The toxic mechanism involves an overwhelming of the liver's detoxification capacity, resulting in hepatocellular necrosis [10]. The severity of liver damage correlates directly with the ingested dose, and an acute overdose can swiftly progress to fulminant hepatic failure [7].…”

“…The first successful liver transplantation (LT) was performed in 1967 by Dr. Thomas Starzl, with a congressional hearing approving LT for general use in 1983 [1]. LT is currently employed as a life-saving measure for multiple indications, including chronic liver disease, hepatocellular carcinoma (HCC), and acute liver failure [2,3]. In 2021, 34,694 LTs were performed globally, a 6.5% increase from 2020 and 20% increase from 2015 [2].…”
