A Cost–Benefit Analysis of Bipolar TURP for the Treatment of Bladder Outflow Obstruction

Abstract: Transurethral resection of the prostate (TURP) is the gold standard surgical technique for endoscopic treatment of benign prostatic hyperplasia (BPH). Introduced in 2001, the bipolar energy appeared to be a valid alternative to the classical monopolar one with reduced risk of complication related to the use of saline irrigation and to the increased hemostatic efficacy. More recently, raising attention has been given to laser enucleation and vaporization techniques, which appear to achieve further advantages in… Show more

“…About 60% of men with BPH—probably and mainly due to the slow onset of LUTS—live with these symptoms, which eventually could result in the deterioration of bladder function, so the physician is consulted only when the damage is irreversible [ 17 , 18 ]. This issue represents a real socio-economic problem, since it has an impact on both QoL of patients and the health care system that supports the costs of this disease [ 18 , 19 , 20 ].…”

“…It is more of a low-cost treatment compared to TURP. Most conveniently, it can be performed in the outpatient operating room (4)(5)(6). Ultrasound-guided transperineal laser ablation (US-TPLA) and prostatic artery embolization (PAE) are new minimally invasive interventional techniques developed in recent years, both of which are suitable for BPH in older people (7,8).…”

“…For several decades, transurethral resection of prostate (TURP) has been recognized as the gold-standard endoscopic treatment for symptomatic (BPH) requiring surgery and those who failed medical therapy and may have complications of outlet obstruction such as bladder stones, urinary retention, or renal insufficiency [ 2 ].…”
