A cut‐off value of basal serum calcitonin for detecting macroscopic medullary thyroid carcinoma

Abstract: Basal serum CT levels higher than 19·0 pmol/l can be a useful cut-off value for detecting macroscopic MTC, even though values below 19·0 pmol/l cannot exclude the presence of MTC like small volume MTC or premalignant C-cell hyperplasia.

“…As we previously described, all US examinations were performed using one of two US systems; an iU22 unit or an HDI-5000 (Philips Healthcare, Bothell, WA, USA) equipped with a linear, high-frequency probe (4 to 15 MHz) [14]. The scanning protocol included both transverse and longitudinal real-time imaging of the thyroid.…”

“…In female patients with calcitonin levels ≥ 35 pg/ml and male patients with calcitonin levels ≥ 46 pg/ml, the risk for MTC is considered to be markedly elevated [20]. Other studies defined 60-65 pg/ml as cut-off values of calcitonin to detect MTC [21,22]. Earlier detection of MTC leads to better biochemical cure rates, as shown by Machens and Dralle in 2015 [17].…”

“…Although MTC is considered a well differentiated tumour, it is quite aggressive [3]. In fact it is the second most aggressive thyroid cancer after anaplastic carcinoma [4], with a mortality rate that ranges between 13, 4 -38% [2][3][4][5]. Its poor prognosis relies not only on its aggressive nature but also on its limited response to chemotherapy and radiation [6].…”
