Background: Study was conducted to analysis efficacy of compression suture in the surgical management of Atonic PPH. It’s prospectively carried out with 60 cases required compression suture in managing Atonic PPH. Atonic uterus is preventable cause of maternal mortality and morbidity constituting 80% PPH cases.Methods: Prospective study carried out at tertiary care center for studying efficacy of compression suture in the surgical management of atonic PPH for one year, started after approval from ethical committee.Results: Compression suture is effective in managing atonic PPH. out of 60 cases, 4 patient's required obstetric hysterectomy as a last resort to control Atonic PPH. Uterus was conserved in 93.3% of cases.Conclusions: Development of compression suture technique has proved to be effective in the control of massive Atonic PPH not responding to medical management is an alternative to hysterectomy when timely applied. It proves to be valuable addition for surgical treatment of PPH and for saving uteri after medical management of PPH failed. Authors can adopt compression suture as a mid-step before resorting to uterine devascularisation or hysterectomy when medical line of management fails. Resident doctors in teaching institutes must be well trained in this technique.

Беломестнов), Екатеринбург, Россия Цель исследования-разработка способа остановки послеродового кровотечения, обладающего улучшенными характеристиками в отношении сохранения анатомической и функциональной целостности матки, а также повышения эффективности лечения и снижения частоты развития осложнений, простоты, быстроты и безопасности выполнения в кризисной ситуации ургентной хирургии. Материал и методы. Предлагаемый способ остановки послеродового кровотечения заключается в том, что со стороны серозной оболочки матки миометрий области плацентарной площадки прошивается сборочным швом, не проникающим в полость матки. Шовный материал захватывает 1 / 3 миометрия области плацентарной площадки и стягивается в поперечном направлении по типу «кисета» одной нитью. Таким образом осуществляется компрессия спиральных артерий плацентарной площадки с минимальной травматизацией окружающих тканей и сохранением кровотока в наиболее крупных ветвях маточной артерии, обеспечивающих основное кровоснабжение мышцы матки. В отличие от известных аналогов предлагаемый гемостатический наружно-маточный надплацентарный сборочный шов не уменьшает объем полости матки. Проведенные клинические испытания предложенного способа остановки послеродового кровотечения у 5 родильниц показали значительное снижение числа гистерэктомий-в 5 раз (р=0,02) и объема кровопотери в 1,7 раза (р=0,03) по сравнению с традиционным подходом (29 родильниц группы сравнения). Заключение. Разработанный способ остановки послеродового кровотечения является эффективным, обеспечивает отсутствие компрессионно-ишемического повреждения миометрия, отличается простотой и быстротой выполнения. Авторы информируют об отсутствии конфликта интересов. Ключевые слова: акушерское гипотоническое кровотечение, хирургический гемостаз, гемостатический наружноматочный надплацентарный сборочный шов на матку.
