A first reported case of clear cell carcinoma associated with delayed extrusion of midurethral tape

Abstract: We present the first reported case of clear cell carcinoma associated with a midurethral tape (MUT), the possible hypotheses and the management pitfalls we encountered. We report a 58-year-old woman who presented with symptoms of urinary tract infection and acute retention of urine associated with vaginal tape exposure 10 years after placement of an inside-out transobturator tape. She subsequently had a partial transobturator tape excision and a diagnostic cystoscopy, which revealed inflammatory changes within… Show more

“…11 Concerns regarding mesh and carcinogenesis stem from historical animal studies, explanted mesh analyses and rare case reports. 24–28 Studies have repeatedly shown that an implant’s physical composition and size rather than its chemical composition determines carcinogenicity. 12 There is a loss of carcinogenicity when implants of various compositions are imbedded in a powder, porous or woven form in animals.…”

“…Similar materials are also used in urogenital surgical procedures, especially in sling techniques for stress urinary incontinence (210). Recently a case of clear cell carcinoma associated with delayed extrusion of mid-urethral polypropylene tape was reported 10 years after insertion (211). The authors' explanation for malignant degeneration was a possible mutation due to longstanding chronic tissue inflammation caused by mesh as a foreign body.…”

“…In the vagina it is often associated with in utero diethylstilbestrol (DES) exposure, while in the urethra the majority of such cancers are associated with a urethral diverticulum. The report by Lin et al [1] describes a patient who presented with urinary retention and symptoms of a urinary tract infection (UTI) who was found upon examination to have mesh exposure in the vaginal fornix and an inflamed and tender anterior vaginal wall. She received a transvaginal tape obturator (TVTo) (Gynecare, Johnson & Johnson, Somerville, NJ, USA) 10 years earlier.…”
