A Framework for the Development and Interpretation of Different Sepsis Definitions and Clinical Criteria

Abstract: Although sepsis was described more than 2,000 years ago, and clinicians still struggle to define it, there is no “gold standard,” and multiple competing approaches and terms exist. Challenges include the ever-changing knowledge base that informs our understanding of sepsis, competing views on which aspects of any potential definition are most important, and the tendency of most potential criteria to be distributed in at-risk populations in such a way as to hinder separation into discrete sets of patients. We p… Show more

“…Gaps in knowledge/critique of evidence The new Sepsis 3 definition has substantially improved construct validity for the concept of sepsis [114]. SOFA is an older tool that predicts mortality in patient populations, although some elements of the SOFA score are outdated (such as "renal dose" dopamine).…”

“…The major difference of these definitions compared to the previous ones was that they were not only based on expert opinion; instead a broad analysis of clinical and laboratory parameters of patients from five large independent cohorts was performed to develop these definitions (5,9). Sepsis-3 definitions are not universally accepted and many controversies have surfaced (10)(11)(12). Clinical data utilized for the development of the Sepsis-3 definitions were mainly recorded in patients hospitalized in US intensive care units (ICU).…”

“…The Critical Care Medicine Journal was equally interested in publishing an article that explains the process of development of interpretation of the new definitions, datailing the knowledge, the purpose and the statistical problem [9]. This favours the idea that when undertaking triage at admission to the ICU using the available biomarkers, it isnot always clear which patients are infected and which are not.…”
