A giant ovarian mucinous tumor in a 58-year-old postmenopausal patient with persistent abdominal pain and high serum levels of CA 19-9

Abstract: Ovarian cancer is the seventh most commonly diagnosed cancer among women in the world and epithelial ovarian cancer is the most predominant pathologic subtype. Tumor markers are widely used in clinical practice to determine therapeutic efficacy, to detect recurrence and to predict prognosis in known cancers. CA-19-9 antigen is mainly elevated in cases of gastrointestinal tract malignancy, including of the pancreas, colorectum, and biliary tract. However, CA 19-9 antigen can also be elevated in ovarian mucinous… Show more

“…Cancer antigen is mostly used as a relatively specific marker for ovarian cancer. According to the literature, the combination of a pelvic tumor and a high level of CA 125 arouses suspicion of a gynecological malignancy, but other benign conditions such as menstruation, endometriosis, and pelvic inflammatory diseases should always be considered in the differential diagnosis, particularly in pre-menopausal women [ 19 21 ]. Although there is limited evidence to support an association between elevated CA 125 levels and uterine fibroids, abnormally high levels have been reported in a small number of patients.…”

“…Epithelial ovarian cancers are considered the most common type of ovarian cancer, accounting for about 90% of cases. More than 75% of epithelial ovarian cancers are of the serous type, whereas mucinous, endometrioid, clear cell, Brenner, and undifferentiated lineage types are less common 3 . Failure in early detection can lead to high mortality rate 3 .…”

“…Therefore, CA199 can be elevated in many types of gastrointestinal cancer such as colorectal cancer, esophageal cancer, and hepatocellular cancer. Moreover, CA199 antigen can also be elevated in ovarian mucinous neoplasms ( 22 ). For perimenopausal women patients, it is also necessary to make a differentiation with regard to endometrial cancer, uterine fibroids, inflammatory conditions, or diseases other than cancers ( 23 , 24 ).…”
