A Hospital Admission System Based on Nursing Work Load

Abstract: A model is presented to compare hospital admission systems based on (i) patient census and (ii) nursing work load. It is shown that nursing costs are lower under the work load-based system than under the patient-based one. Sensitivity studies show that the difference between the two systems is due to the fact that the work load-based policy takes into account patient differences.

“…The models described above all focus on maximizing the utilization of bed resources, which can lead to extreme variations in the utilization of other resources. Offensend [82] was the first author to develop a hospital admission system based on nursing work-load requirements. In his research, a simulation model was used to compare occupancy-related admission policies with systems that considered the relationship between admissions and nursing requirements.…”

“…Simple Markov decision models have been proposed earlier for the analysis of admission p~licies.~, A first attempt to regulate care demand through the control of admission has been described by Offensend. 6 In References 2 and 3, a more comprehensive model based on the description of a care unit as a population process has been proposed, and a heuristic solution has been obtained. In this model, the total care demand in the care unit is a function of the population census, i.e.…”

“…Citons en particulier le problème du contrôle des admissions de patients dans un service hospitalier. Kolesar [12], Offensend [15] et Duguay et Haurie [4] ont utilisé un modèle de chaîne de Markov commandée pour analyser îe flot de patients dans une unité de soins. Cependant, des hypothèses très restrictives sur la dynamique des soins pour chaque patient ont dû être faites dans chacune de ces études.…”
