A Multinational Outbreak of Histoplasmosis Following a Biology Field Trip in the Ugandan Rainforest

Abstract: This is an unusual outbreak of histoplasmosis following short-term travel to Africa. Pulmonary histoplasmosis should always be considered in the differential diagnosis of an acute febrile respiratory illness in travelers returning from endemic areas or reporting activities suggesting exposure.

“…3 Although histoplasmosis occurs in Uganda, the overall risk is not well understood. In 1970, a study of skin sensitivity to histoplasmin, including a total of 1,144 subjects and roughly equal proportions of adults and children, was conducted in six regions of Uganda.…”

“…H. capsulatum is highly endemic in the Americas, but cases imported by travelers are almost all from Latin America (Central and South America) and are much more rarely reported from North America despite the fact that some regions of the United States are highly endemic. Histoplasmosis also exists in other regions of the world such as East Africa, 11,13 the Indian subcontinent, 14,15 and Australia, 16 but there are hardly any cases reported by travelers to these regions. 17 The reason for this pattern is unclear, but we suggest two possible explanations.…”

“…4,7 In recent years, a growing number of cases of histoplasmosis, both sporadic and in clusters, have been reported in travelers. 4,6,[8][9][10][11] This rise in apparent cases of histoplasmosis may be the result of increased international travel, and probably increased awareness among medical practitioners.…”

“…недавно описаны случаи индигенного гистоплазмоза в Южной корее [9]. заболевание выявлено в некоторых африканских странах, преимущественно у вич-инфицированных [10,11]. аутохтонные случаи гистоплазмоза в европе редки, однако зафиксированы в испании, италии, германии, турции [12,13].…”
