A novel, low-cost, reusable, high-fidelity neurosurgical training simulator for cerebrovascular bypass surgery

Abstract: OBJECTIVE Cerebrovascular bypass surgery is a challenging yet important neurosurgical procedure that is performed to restore circulation in the treatment of carotid occlusive diseases, giant/complex aneurysms, and skull base tumors. It requires advanced microsurgical skills and dedicated training in microsurgical techniques. Most available training tools, however, either lack the realism of the actual bypass surgery (e.g., artificial vessel, chicken wing models) or require special facilities and regulations (e… Show more

“…A solution around this is the use of low-cost, high-fidelity solutions for dissection. These include the use of veterinary cadavers for spinal dissection, use of gelatin and silicone to simulate cerebrovascular surgery dissection, and the use of phone cameras as operative microscopes (26)(27)(28). These cheaper and innovative solutions can bridge the training gap in African neurosurgery programs.…”

“…Taken together, the material cost for an MCA model is less than $5. This price is substantially cheaper than the price of other effective models, and our model is far easier to replicate when compared to the one made by the University of Washington [ 19 ]. While the internal mold is dissolved in chloroform to acquire a hollow model, the external mold, however, can be used repeatedly.…”

“…Although endovascular procedures have become the more commonly used, surgical management still remains strongly indicated in Moya Moya disease, management of complex aneurysms among other conditions (Belykh et al, 2016;Cikla et al, 2018;Colpan et al, 2008;Shi et al, 2015). Aneurysms are a relatively common neurosurgical pathology whose management in many centers is largely endovascular with coiling or stenting.…”

“…However, it has been largely underexplored in neurosurgery until recently (Ploch et al, 2016) Most anatomy textbooks and atlases and most radiological investigations like CT and MRI offer a 2D representation of anatomy which is difficult to visualize and correlate with intraoperative anatomy (Shah et al, 2018;Panesar et al, 2019). Recent studies have even argued that anastomosis training models are superior to the traditional apprenticeship training (Cikla et al, 2018). As the 3D printing software and machines become more available and affordable, many centers have a well-established 3D printing system used for various activities including teaching, prosthesis creation etc.…”
