A Prospective Comparison of 3 Approved Systems for Autologous Bone Marrow Concentration Demonstrated Nonequivalency in Progenitor Cell Number and Concentration

Abstract: The Harvest system resulted in a greater CTP number and concentration after centrifugation when compared with the Biomet and Arteriocyte systems and may thus provide increased osteogenic and chondrogenic capacity.

“…Hegde et al compared the Magellan to two other FDA-approved bone marrow concentration systems, namely Harvest SmartPrep 2 (Harvest Technologies Corporation, Plymouth, MA) and Biomet BioCUE (Biomet Biologics, Warsaw, IN), for their capacity to isolate progenitor cells from BMA of iliac crest in humans. The SmartPrep 2 system resulted in a greater concentration of progenitor cells, evaluated by means of a CFU-F assay, when compared to the Biomet and the Magellan system [25]. Cassano et al performed extensive comparison on the composition of human BMC and platelet-rich plasma using also the Magellan and the SmartPrep 2.…”

“…The BMC may also be prepared manually in a sterile glass or plastic blood-collection tube with a closure that is evacuated to create a vacuum inside the tube, facilitating the draw of the predetermined BMA; however, no data exists comparing mononuclear cell counts using either the centrifuge or blood-collection tube method. As mentioned previously, it is now known that the various commercially available centrifuge systems may not generate equivalent amounts of HSCs and progenitor cells; however, this may be due to differences in the bone marrow acquisition process [77]. Kon et al recently presented data at the International Society of Arthroscopy, Knee Surgery and Orthopaedic Sports Medicine (ISAKOS) 2013 meeting reporting that BMC cell counts are superior to those of BMA; however, clinical superiority remains polemical [78].…”

“…33 Using more contemporary techniques such as flow cytometry to simultaneously evaluate several cell surface markers in the aspirate as opposed to separated and cultured cell populations, it is more relevant to state that a standard 4 to 6 mL of BMC generated from centrifugation of 60 mL of BMA will contain approximately 40,000 MSCs. 19,26,34,35 Defining the actual number of MSCs in BMA is difficult because there is no one set of cell surface markers that can identify the various subpopulations of MSCs in vivo. Great caution should be used when relying solely on CFU to determine or to compare the number of MSCs between products.…”
