A Prospective Evaluation of Femoral Tunnel Placement for Anatomic Anterior Cruciate Ligament Reconstruction Using 3-Dimensional Magnetic Resonance Imaging

Abstract: Despite contemporary techniques and a concerted effort to perform anatomic ACL reconstruction by 4 experienced sports orthopaedic surgeons, the position of the femoral footprint was significantly different between the native and reconstructed ACLs. Furthermore, each surgeon used a different technique, but all had comparable errors in their tunnel placements.

“…Similarly, the accuracy of surgical placement is seen to vary about a planned point. In a study by Hart et al, four different "anatomical" techniques were compared with a mean error in placement of 3-4 mm with each method [20]. Thus, there is variation in both anatomy and surgical accuracy (both in the region of 4 mm) equating to combined errors that may be relatively large (8 mm).…”

“…The imaging protocol for this study was a previously validated isotropic 3D MRI protocol. 19,21,27 Both 2D and 3D MRI scans were performed by use of the same 1.5-T Twin-Speed Excite high-definition MRI scanner (GE Medical Systems). In the scanner, the knees were positioned in near full extension, and an 8-channel high-definition surface coil was applied.…”

“…Type 2 patients, for example, to better ascertain how an OTT revision compares to two-stage revision ACL procedures). Additional efforts will also aim to assess feasibility across other diagnostic modalities, such as MRI, 3D-MRI, 51 arthroscopic examination videos, and 3D printed models. This will be important going forward to ascertain the need for CT in this classification based on its performance with other modalities, given the risks of radiation.…”
