HIV infection and undernutrition remain significant public health concerns for infants and children. In infants and children under these conditions, undernutrition is one of the leading causes of death. Proper management of nutrition and related nutrition complications in these groups with increased nutrition needs are prominent challenges, particularly in HIV-prevalent poor-resource environments. Several studies support the complexity of the relation between HIV infection, nutrition, and the immune system. These elements interact and create a vicious circle of poor health outcomes. Recent studies on the use of probiotics as a novel approach to manage microbiome imbalance and gut-mucosal impairment in HIV infection are gaining attention. This new strategy could help to manage dysbiosis and gut-mucosal impairment by reducing immune activation, thereby potentially forestalling unwanted health outcomes in children with HIV. However, existing trials on HIV-infected children are still insufficient. There are also conflicting reports on the dosage and effectiveness of single or multiple micronutrient supplementation in the survival of HIV-infected children with severe acute malnutrition. The WHO has published guidelines that include time of initiation of antiretroviral therapy for HIV-pregnant mothers and their HIV-exposed or HIV-infected children, micronutrient supplementation, dietary formulations, prevention, and management of HIV therapy. However, such guidelines need to be reviewed owing to recent advances in the field of nutrition. There is a need for new intervention studies, practical strategies, and evidence-based guidelines to reduce the disease burden, improve adherence to treatment regimen, and enhance the nutrition, health, and well-being of HIV-infected infants and children. This review provides up-to-date scientific information on current knowledge and existing challenges for nutrition therapy in HIV-infected infants and children. Moreover, it presents new research findings that could be incorporated into current guidelines.

Aim: The aim of our study was to study and compare the impact and efficacy of medical nutrition therapy (MNT) with that of the standard nutrition therapy(SNT) in children diagnosed with Severe thiness in the age group of 5-10 years and diagnosed as severely thin. Study Design: It was a prospective comparative study, conducted over a period of 18 months at the Nutrition Rehabilitation, Research and Training Centre (NRRTC) associated with a tertiary care hospital in India. Methods and Materials: A total of 113 children in the age group of 5 to 10 years with severe thinness were enrolled and divided into 2 groups—58 were placed in the MNT group and 55 in the SNT group for a period of 8 weeks. The children in both groups received MNT and SNT diet over the said period of 8 weeks. Primary outcome variables were weight gain and body mass index (BMI) (i.e., change in nutritional status). Results: At the end of 8 weeks, the rate of weight gain (gm/kg/day) was significantly greater in the MNT group, that is, 2.35 gm/kg/day as compared with the SNT group, that is, 0.73 gm/kg/day, and the change in nutritional status was significantly better in the MNT group (p < 0.001) with 62.1% achieving normal nutritional status, 32.8% remaining thin, and 5.2% remaining severely thin as compared to the SNT group in which 9.1% achieved normal nutritional status, 65.5% remained thin, and 25.5% remained severely thin. Conclusion: To conclude, MNT is superior to SNT for the treatment of severe thinness (ST).

Aim and objectives To study variations in biochemical and nutritional profile among children with severe thinness receiving medical nutrition therapy. Methods This study was conducted in 52 children of age between 5 and 12 years in Paediatric ward at Tertiary Care Centre over the period of 18 months. Serial anthropometry and blood collection was done on enrolment, 8 weeks and 6 months. Samples were analysed for serum lipid profile, calcium, magnesium, malondialdehyde and vitamin C levels. Results On enrolment all subjects had insufficient vitamin C levels, increased malondialdehyde levels, micronutrient deficiencies and altered lipid profile with less than desirable cholesterol, borderline HDL, severely low LDL and acceptable triglyceride levels. Subjects were started on MNT, i.e. indigenously prepared Ready-to-use therapeutic food. At 8 weeks, MNT improved vitamin C levels (91.1%) and 80% subjects had normalised malondialdehyde values and improvement in micronutrients with cholesterol (desirable in 44.4%), HDL (desirable in 51.1%), LDL values (acceptable in 24.4%) and triglyceride (high in 20.5%). At 6 months, subjects maintained vitamin C values to sufficient (81.5%) with normalising of MDA in 81.8% of subjects and with cholesterol (desirable in 55%), HDL (desirable in 33.1%) LDL (acceptable in 22.1%), and triglyceride (acceptable in 60%) and maintaining of micronutrients within normal ranges. 33.33% subjects showed improvement to thinness at the end of 6 months, while 44% subjects became normal by the end of study. Conclusion MNT or Ready-to-use therapeutic food (RUTF) is effective in treating acute malnutrition and promoted as the treatment of choice for children in age group of 5–12 years.
