Background: Intussusception in older children may be less common than intussusception in infants. However, it is a significant cause of bowel obstruction across all age groups. The aim of this study was to evaluate our experience with regards to older children (5 years and above) who were managed for intussusception in a single tertiary center in a developing country. Materials and Methods: This was a retrospective study of children, aged 5 year and above, who presented with intussusception (ultrasound confirmed) over a 5-year period at the pediatric surgery unit of a teaching hospital in Enugu, Nigeria. The information analyzed included the patients’ age (in years), gender, time interval between onset of symptoms and presentation to the hospital, presenting symptoms, operative procedure performed, complications of treatment and outcome of treatment. Results: A total of 378 cases of childhood intussusception were seen during the study period. Out of this number, 18 (4.8%) patients were older than 5 years of age. There was male predominance. Abdominal pain was the most common and consistent symptom in all the patients and majority of the patients had bowel resection (right hemicolectomy with ileotransverse anastomosis). One-fifth of the patient had pathologic lead point (PLP) Wound infection was the most common post-operative complication and one patient expired due to overwhelming sepsis. Conclusion: Although intussusception is more common in infants; it’s occurrence in older children is not uncommon and may be associated with PLP. Non-operative treatment is often ineffective and operative treatment is always required.

Introdução: A intussuscepção intestinal é uma das principais causas de obstrução intestinal em crianças. Em pacientes acima de cinco anos, em geral, é secundária a lesões predominantemente benignas, entretanto, os tumores malignos intestinais também podem causar invaginação intestinal. O adenocarcinoma de cólon é um dos cânceres mais frequentes em adultos, porém é raro em pacientes pediátricos. Os pacientes apresentam subtipos mais agressivos e em estágios mais avançados e tem sobrevida média de 26 meses. Relato do caso: Neste trabalho é relatado o caso de um paciente masculino com 12 anos de idade diagnosticado com invaginação íleo-ceco-cólica por adenocarcinoma de ceco. Conclusão: A invaginação intestinal em pacientes acima de cinco anos está fortemente associada a pontos patológicos de invaginação. Causas benignas são mais frequentes, porém, dada a agressividade do adenocarcinoma de cólon nas crianças e nos adolescentes, deve-se estar atento para os sintomas de alarme com alto índice de suspeição, permitindo o diagnóstico precoce e o tratamento oncológico adequado.

Background: Intestinal intussusception is the inversion of a portion of the proximal part of intestinal loop into the distal part of the loop. It leads to intestinal obstruction and vascular compromise. This predicament is rare in adults and is usually associated with benign or malignant neoplasms but very rarely with Gallbladder polyps. Case presentation: We present a case of a 41 year old male, presenting with abdominal pain, alternating episodes of diarrhea and constipation and gaseous abdomen for the preceding 5 months. Patient had pallor of mucosa, no jaundice, normal scaphoid shaped and non-tender abdomen and unremarkable head and neck. On complete blood count (CBC) the patient had decreased hemoglobin and raised lymphocytes. Multiple gut intussusceptions were observed in 4 areas and multiple echogenic foci were also observed in the gallbladder. Laparoscopic cholecystectomy showed a densely adherent gallbladder. On exploratory laparotomy no intussusceptions were seen at the indicated areas but rather abnormal constrictions of the gut which indicated past intussusceptions, suspected to have spontaneously healed without intervention. Conclusion: Intestinal Intussusceptions rarely occur in adults and are almost always accompanied by benign or malignant neoplasms. They rarely with illnesses such as gallbladder polyps. However, possibilities remain so these should not be ruled out without proper investigations. Proper follow up should therefore, be done owing to the possibility of recurrence.
