A Rare Case of Systemic Adult Burkitt Lymphoma Presenting as Acute Acalculous Cholecystitis

Abstract: Acute acalculous cholecystitis (AAC) is an uncommon presentation of cholecystitis accounting for 10% of cases. AAC is caused by a localized ischemic event in the gallbladder usually in critically ill patients. Several cases of localized or systemic lymphoma have been described in the literature as causes of AAC. We present a patient with a rare case of AAC due to undiagnosed systemic Burkitt lymphoma. Pathology of the gallbladder revealed Burkitt lymphoma with analysis of his cerebral spinal fluid confirming S… Show more

“…Malignant lymphoma of the gallbladder is a rare form of gallbladder malignancy, which accounts for 0.1%-0.2% of all gallbladder cancers[ 1 - 3 ]. In previous reports, most of these malignancies were documented to be diffuse large B-cell lymphomas and MALTomas[ 2 , 4 ], and only two reports had documented Burkitt’s lymphoma of the gallbladder[ 6 , 7 ]. Compared to previous cases, the present case yielded some interesting clinical and imaging findings.…”

“…In almost all cases of this malignancy, patients are diagnosed as having gallbladder adenocarcinoma or cholecystitis at the time of surgery, and preoperative diagnosis is extremely difficult. Although several reports have documented malignant lymphomas of the gallbladder, most of these malignancies are diffuse large B-cell lymphomas and mucosa-associated lymphoid tissue-type lymphomas (MALTomas)[ 2 , 4 , 5 ], and only two reports have previously documented Burkitt’s lymphoma of the gallbladder[ 6 , 7 ]. Herein, we report the case of a patient with Burkitt’s lymphoma of the gallbladder mimicking gallbladder adenocarcinoma.…”

“…Primary lymphoma of the gallbladder is rare but likely fits on the spectrum of lymphomas occurring in the gastrointestinal tract[ 5 , 7 ]. There are several case reports of acute cholecystitis as the initial presentation of lymphoma of the gallbladder and all describe non-Hodgkin's lymphoma or its subtypes on histopathology[ 2 - 5 ]. On the contrary, our case report describes Hodgkin’s lymphoma of the portal lymph nodes, and benign pathology of the gallbladder itself, presenting as ruptured cholecystitis and mimicking gallbladder adenocarcinoma on imaging.…”

“…Other authors have written about clinical situations where there is a mass in the region of the gallbladder or biliary ducts causing acute acalculous cholecystitis. In these settings, the distinction between lymphoma and gallbladder adenocarcinoma relies on histopathological assessment[ 2 ].…”

“…That is because preoperative diagnosis is exceedingly challenging. Although several reports have documented malignant gallbladder lymphomas, most of these malignancies are diffuse large B-cell lymphomas (DLBCL) or Marginal Zone Lymphomas (MZL) [3,[5][6], and only three reports have previously documented gallbladder Burkitt's lymphoma (BL) [1,[7][8]. Herein, we report the first case of a child with gallbladder BL.…”
