A Rare Complication of Electric Shock: Myocardial Infarction

Abstract: Myocardial infarction is one potential outcome after an electric shock although it is seen relatively rarely and its pathogenesis remains controversial. Coronary arteriography is the most helpful investigation in assessing the physiopathology of the rare event. These patients merit a careful scrutiny due to an increased death rate because of cardiopulmonary arrest. Here, we reported a man with inferior myocardial infarction following electrical shock. Although he had frankly normal coronary arteries by coronar… Show more

“…It is stated in the literature that right coronary artery lays closer to the chest wall on its course and it is more sensitive to electrical current, so ST elevation in inferior derivations is more frequently observed after electric shock [ 14 ]. Al et al determined normal coronary arteries after electric shock induced inferior MI and they reported that the patient's ECG findings were recovered after 6 hours [ 15 ]. Celebi et al also presented an electric shock induced inferior MI patient with normal coronary arteries.…”

“…Electric shock induced multiple traumas and/or hematomas may also cause contraindication for fibrinolytic treatment. According to all of these, in such cases first treatment option should be percutaneous coronary angioplasty [ 15 , 16 ].…”

“…Kardiyak troponin düzeylerinin pulmoner emboli, ciddi konjestif kalp yetmezliği, direkt kardiyak yaralanma, elektrik çarpma-sı, toksin etkisi (adriamycin, 5-fluorouracil, trastuzumab, yılan zehiri), viral miyokardit, perikardit, serebrovasküler olay, sepsis, taşiaritmiler ve lober pnömoni gibi non-iskemik durumlarda, kronik obstrüktif akciğer hastalıklı (KOAH) olgularda da yükseldiği bildirilmiştir (4,5). KOAH endüstriyel topluluklarda hala artmakta olan yegane mortalite nedenidir.…”
