This paper highlights the distinction between precursor and control variables in the context of reasoned action theory. Here the theory is combined with structural equation modeling to demonstrate how age and past sexual behavior should be situated in a reasoned action analysis. A two wave longitudinal survey sample of African-American adolescents is analyzed where the target behavior is having vaginal sex. Results differ when age and past behavior are used as control variables and when they are correctly used as precursors. Because control variables do not appear in any form of reasoned action theory, this approach to including background variables is not correct when analyzing data sets based on the theoretical axioms of the Theory of Reasoned Action, the Theory of Planned Behavior, or the Integrative Model

ObjectiveTo understand why skilled birth attendance—an acknowledged strategy for reducing maternal deaths—has been effective in some settings but is failing in Pakistan and to demonstrate the value of a theory-driven approach to evaluating implementation of maternal healthcare interventions.DesignImplementation research was conducted using an institutional ethnographic approach.Setting and populationNational programme and local community levels in Pakistan.MethodsObservations, focus group discussions, and in-depth interviews were conducted with 38 Community Midwives (CMWs), 20 policymakers, 45 healthcare providers and 136 community members. A critical policy document review was conducted. National and local level data were brought together.Main outcomesAlignment of programme theory with real-world practice.ResultsData revealed gaps between programme theory, assumptions and reality on the ground. The design of the programme failed to take into account: (1) the incongruity between the role of a midwife and dominant class and gendered norms that devalue such a role; (2) market and consumer behaviour that prevented CMWs from establishing private practices; (3) the complexity of public–private sector cooperation. Uniform deployment policies failed to consider existing provider density and geography.ConclusionsGreater attention to programme theory and the ‘real-world’ setting during design of maternal health strategies is needed to achieve consistent results in different contexts.

Logical operating model of the hearing conservation program for workers

Purpose: to validate the contents of an operating model of hearing conservation programs. Methods: this is a documentary and methodological research, in which the operating model was evaluated by ten experts in audiology, through the analysis of the activities developed in the implementation of the hearing conservation program. The components were analyzed according to the formula that determines the content validation index by item, and the operational logical model as a whole, according to the content validation formula by scale level. The items were considered valid when agreement among participants reached more than 80%. Results: the operating model was structured in 4 (four) dimensions, namely: (1) Management; (2) Environmental Control; (3) Attention to Hearing Health; (4) Evaluation of Efficacy and Efficiency. Each dimension corresponds to a set of activities (referred to as processes), and their respective results expected. For the structure of the program, considering its set of dimensions, material, organizational and human resources were listed. Conclusion: the proposed operating model and its components proved to be valid in their relevance and comprehensibility, offering support and applicability in evaluating the effectiveness and efficiency of the program.
