A whole-genome association study of major determinants for allopurinol-related Stevens–Johnson syndrome and toxic epidermal necrolysis in Japanese patients

Abstract: Stevens-Johnson syndrome and toxic epidermal necrolysis (SJS/TEN) are severe, cutaneous adverse drug reactions that are rare but life threatening. Genetic biomarkers for allopurinol-related SJS/TEN in Japanese were examined in a genome-wide association study in which Japanese patients (n=14) were compared with ethnically matched healthy controls (n=991). Associations between 890 321 single nucleotide polymorphisms and allopurinol-related SJS/TEN were analyzed by the Fisher's exact test (dominant genotype mode)… Show more

“…Thus, screening for the HLA-B*5801 allele has been proposed to reduce the incidence of these nonimmediate DHRs [33][34][35]. Another promising approach is to genotype the rs9263726 variant in psoriasis susceptibility 1 candidate 1 gene as a surrogate marker [36] that is in absolute linkage disequilibrium with HLA-B*5801 [37].…”

“…For example, certain HLA-types such as, HLA-B*1502, HLA-A*3101 and HLA-B*5801 have been to be linked with TEN development when exposed to specific drugs. [17][18][19] As sepsis is the most dreadful complication, drugs that are effective against multi drug resistant bacteria are commonly administered. Currently, no treatment modality has been established as a standard for these patients.…”

“…Hung et al [8] published the first report of a strong association between allopurinol-induced severe cutaneous adverse reactions [including StevensJohnson syndrome (SJS) and toxic epidermal necrolysis (TEN)] and human leukocyte antigen (HLA)-B*58:01 in Han Chinese patients. A strong association between HLA-B*58:01 and allopurinol-induced SJS and TEN has been reported in Japanese patients [9]. Furthermore, druginduced liver injury (DILI) is the most frequently reported adverse drug event in Japan [10].…”
