Acanthamoeba keratitis related to orthokeratology

Abstract: The risk of Acanthamoeba keratitis as a potential complication of overnight orthokeratology should be considered, especially in patients with over one-year duration of contact lens wearing. Careful contact lens management is needed and tap-water rinsing should be eliminated from the lens care regimen.

“…Therefore, PHMB is considered to be the first-choice therapy for Acanthamoeba keratitis. Numerous studies proved acanthamoebicidal efficacy and good clinical outcome with prompt local treatment [39,40,[88][89][90][91][92][93][94][95][96][97][98][99][100][101][102] . On the other hand, treat-22 ment failures have been reported, particularly when presentation was late, in cases of deep stromal infection, or in rare cases of primary PHMB resistance of Acanthamoeba strains [41,42,96,103] .…”

“…Two patients had been daily wearing soft contact lenses for several years and the other had worn orthokeratology contact lenses for approximately 1.5 yr. Number of young people wearing contact lenses has been increasing in Korea for correction of myopia and even for cosmetic purpose [32,33]. More attention should be paid to hygienic maintenance of contact lenses care system for prevention of AK [34].…”

“…1 The cornea becomes hypoxic with lid closure during sleep, leading to the epithelium becoming swollen and hence more prone to abrasion. 2,3 Most cases of orthokeratology-associated microbial keratitis were reported to be caused by Pseudomonas aeruginosa. 4 It is becoming apparent that Acanthamoeba is one of the primary causes of keratitis after orthokeratology.…”

“…5 Especially, the risk of Acanthamoeba keratitis is increased in patients with .1-year duration of contact lens wearing. 3 Most reported cases of Acanthamoeba keratitis after orthokeratology have involved a wearing period of .1 week. [3][4][5][6][7] The authors report a case of bilateral Acanthamoeba keratitis after wearing orthokeratology lenses for only 5 days.…”
